Caught in, under or between a moving and a stationary object

Injured was poking the built-up coal out of the chute to the transfer screw using a 1/4"" piece of round stock steel. The poker made contact with the screw conveyor below, causing it to pinch their finger and cause a laceration on the ring finger of their left hand.

Struck by... NEC

Millwright was loosening the bottom nut on the coal mill journal. When pulling to loosen the nut, it suddenly broke free and the wrench slipped off and struck the inside of EE's left knee.

Over-exertion in lifting objects

Kiln Assistant was moving an oxygen tank from the storage rack to the CAT 908 loader bucket for transportation. When EE bent over to lift up the bottom end of the tank, EE felt a pull/pop in the outside of EE's right hamstring.

Struck against a moving object

Injured reports that the haul truck EE was operating was jarred while being loaded which caused EE's shoulder to strike the inside of the door. Injured did not report incident until the following day. Medical treatment was provided and work restrictions were given.

Injured was assisting another employee clean hydrate slurry from a 3' deep bucket elevator pit. One employee left the area for a few minutes. When they returned, injured was standing in the slurry in the pit wearing jeans and standard work boots. Later that evening, they began having leg irritation and sought medical treatment the following day.

Injured was attempting to remove hydrator discharge pipe to clear blockage. After opening pressure relief valve and loosening pipe union, residual pressure caused hydrate material to spray into EE's eyes.

Over-exertion NEC

Injured was attempting to get product to flow from silo into truck using a bar when ee felt pain in shoulder. Incident occurred on 1/29, reported initially on 1/31, and requested medical attention on 2/5 when condition didn't improve.

Fall to the walkway or working surface

Employee slipped on ice/snow and fell in roadway between kilns and hydrate building at ground level. Slip and fall resulted in an ankle sprain.

3 employees were installing bushing on quarry hammer. While pounding on a plate to get bushing into place employee swinging hammer hit plate. Hammer slipped from plate hitting employee in mouth and shoulder.

Injured was using a punch and hammer to set a ring seal in a pump head. While striking the punch, the hammer slipped off the punch and struck left hand.

Injured was assisting another employee replacing a steel plate in the crusher. Employee handed plate to injured and let go thinking injured had it securely. Plate rotated and struck injured's ear causing laceration.

Struck by falling object

Injured was using an air chisel to remove ash ring from #2 kiln when a piece of ash fell and struck shoulder/arm. Injured did not request medical treatment until 2/16/2017.

Over-exertion in lifting objects

Employee was lifting and pulling out cut metal from hopper when he felt pain in his left wrist. Injured reported the incident on 8/31 but did not request medical attention until 9/3.

Over-exertion in wielding or throwing objects

Injured was using a hammer to remove a link from a chain when he felt pain in his lower back. Injured sought treatment after work when the condition didn't improve.

Caught in, under or between a moving and a stationary object

Injured was removing rotary airlock from baghouse. While moving airlock, his hand was pinched causing a laceration.

Absorption of radiations, caustics, toxic and noxious substances

Injured was attempting to re-light kiln when a puff of lime dust blew out of the hood and into his eyes. Injured was wearing safety glasses but not face sheild as required.

Injured was sweeping in the screening building when gust of wind blew dust into his eyes behind is safety glasses.

Over-exertion NEC

Injured stepped from the bottom step at the bagging machine to the concrete floor and felt a pop in his right knee. Injured requested medical attention later in the shift and was given work restrictions.

Employee was using a portable cutting tool to cut 1.5 inch metal rod. The cutting wheel blade broke and caused a laceration to the employees right forearm.

Struck against stationary object

Employee was attempting to unplug a chute, while attempting to break the material free the employee struck his forearm on the top of the chute. The employee did not immediatley report the incident to management, the accident was deemed recordable on June 7th, 2010.

Over-exertion NEC

While the employee was walking from his work location to the rest room he came in contact with a patch of ice. The employee slipped, but did not fall and reported a pain in his hip. On 2/9/2010 the employee requested medical attention and the injury was diagnosed as a hip strain/contusion and the employee has been placed on restricted duty until a follow up exam can be performed.

Fall to lower level, NEC

Employee (electrician) was working on Rail Scale project and was moving a piece of conduit. He slipped on snow and fell into an 18"" trench, causing soreness to his left ankle and calf. Diagnosed with sprain/strain.

Injured was attempting to disolve a plug-up in a line using muriatic acid. The acid spalshed onto the employee's shoulder causing a small 2nd degree burn.

Struck by flying object

Injured was preparing to weld anchors to the kiln shell and was cleaning dust from the area. When the employee cleaned the dust from the area, airborne dust entered his right eye.

Injured was using an impact wrench, with a 4 inch socket, to remove a nut from the axle of the plant dump truck. The socket broke and struck the injured's left index finger, causing a laceration.

Over-exertion in lifting objects

Employee was shoveling lime under conveyor by Robo Bagger-he felt a pull and twinge in lower back. On September 13, 2007 still has pain-Had MRI and found ruptured disc.

Injured was closing a damper on the M tubes when he inadvertently let go of the handle on the chain binder. When the handle swung back, it struck the injured's chin causing a laceration and 2 chipped teeth.

Injured was shoveling by conveyor, began experiencing lower back muscle spasms.

Injured was attempting to unplug a blocked chute with a hammer when he struck his thumb with the hammer.

Injured was replacing expansion joint for the kiln ID fan ductwork. Injured was using a hammer and chisel when a small piece of the chisel broke free and caused a minor laceration to left hand. Injury occurred on 9/28/06, but was not reported to management until 10/2/06 when laceration became infected.

While he was working on #73 elevator, he was shoveling up lime. When he twisted he got a sharp pain in his back.

While outside checking the stone silos, Employee stepped wrong and twisted his left ankle.

Struck by falling object

Put tool back on hook improperly. Tool vibrated off the hook from the vibrations of the machine and hit his left calf.

While walking outside, employee stepped on a stone and twisted his right ankle and his right achilies tendon near the quicklime silos.

When he went to shut off the scraper engine, he climbed up on the tire and hit his right knee on the toe hook.

The pump blocked up and he went to unblock it. It was hydrating inside pump and it blew up in his face giving him burns from the lime in both his eyes.

Fall from machine

While changing a belt on the conveyor, he fell 4 feet through the belt and hit his left elbow and both shoulders and his chest. Did not need immediate treatment.

While employee was cleaning up a pile of lime, lime got into his boots causing a lime burn to the lower portions of both legs.

While employee was bagging lime, one of the full bags of lime blew up and lime got into his left eye and burnt it.

While trying to release an air valve in the Hydrator Compressor Room, employee jumped up to reach the valve and landed wrong, straining his left knee.

While EE was fixing the pump on the overflow tank because it was plugged up, he put it back in the tank after, and the hose/clamp let loose giving him lime burns to his left forearm.

While EE was using a sledge hammer to clear the hopper repeatedly throughout the day, he hurt his lower back.

While employee was walking across the parking lot to get to car to go home, employee slipped on the ice and sprained right ankle.

While taking a broken flange off of a paper thin pipe, his hand slipped and he lacerated his right index finger about 1 1/2 centimeters long.

While replacing a belt for the kiln on top of the number six tank, he pulled the belt to get the ends together to splice, he pulled the muscles in his lower back.

Glenn was working on a project of replacing a conveyor in our Quarry. He was climbing a 3-foot, 3-rung ladder on the east end of the new conveyor. His left foot was on the top of the platform and his right foot was on the top rung of the ladder when the ladder slipped out, he could feel his left knee twist, before he caught his balance.

EE was cleaning up a pile of lime. He was shoveling and strained his lower right side of his back.

While cleaning the eye for the Kiln temperature gadget, he stepped back too far, slipping and fell on his left leg and hyper-extended his right leg.

While fixing the dust collector, the handrail broke as he was standing up, causing a gash on his left thumb and hyper-extending his left elbow.

While he was poking the chute out for the #2 Kiln and then the #1 Kiln, the bucket slipped out of his hands (30-40#)and scraped his left shin leaving an abrasion.

While putting in coal, he lifted wrong and strained his lower back. (EE WAS SHOVELING COAL INTO COAL TANKS.)

While filling quicklime bags, a piece of quicklime flew behind his safety glasses into his left eye.

While doing maintenance to one of the pickup trucks, he hit the shaft with a hammer and a chip broke off the shaft, leaving a 1/2"" laceration on his right cheek on his face and a small piece of metal got imbedded.

Doing maintenance on dust tank, bottom floor. Assisting with another employee pulling a 48"" x 68"" x 3/8"" sheet of metal through a 5' x 6' opening in the wall of the building. A crane was supporting the load as it was being pulled into the building. While holding the sheet of metal, James felt a a sharp pain in left shoulder.

EE WAS WORKING WITH CO-WORKER TAKING COAL MILL APART FOR #2 KILN. THEY WERE IN THE PROCESS OF REMOVING A PLATE WEIGHING ABOUT 150 LBS. EE WAS REMOVING PIN HOLDING THE PLATE, WHILE CO-WORKER WA S SUPPORTING THE PLATE. THE PLATE SLIPPED HITTING INJURED EE'S LEFT HAND, TIPS OF THE MIDDLE AND RING FINGERS.

Over-exertion NEC

WHILE WALKING ON THE BACK SIDE OF THE HAMMER MILL SCREW ON BOTTOM OF CORSON BUILDING, EE SLIPPED ON THE WET FLOOR. TRYING TO CATCH HIMSELF HE REACHED UP AND GRABBED THE CATWALK ABOVE FOR THE H AMMER ACCESS AREA. THIS CAUSED A STRAIN OF THE RIGHT SHOULDER.

Fall from ladders

WHEN STEPPING OFF THE BAGGER, EE FELT A TWINGE IN HIS LEFT KNEE. ON JUNE 18TH HE WAS BEING TREATED FOR ARTHRITIS OF THE LEFT KNEE AND BURSITIS IN THE LEFT HIP. NULY 2ND EE HAD A MRI DONE, FINI DNG A TEAR IN A LIGAMENT OF THE LEFT KNEE. HE WAS TO BE OPERATE ON JULY 8TH BUT FOUND OUT HE HAD AN HEART CONDITION THAT HAD TO BE TREATED FIRST. HE WILL HAVE THE KNEE OPERATION ON JULY 30TH

EE WAS REPAIRING ""BECK"" ON TOP OF BAGHOUSE.THEY WERE FINISHED AND WERE CLEANING UP AND PACKING UP THIER TOOLS. THE TOOLS WERE PLACED IN A 5 GALLON BUCKET AND AN AIR CHISEL WAS STICKING OUT 13"". EE TRIPPED ON WALKWAY THAT WAS LIFTED FOR THE WORK AND FELL ONTO THE BUCKET AND CHISEL. STOMACH AREA WAS SORE AND TENDER.

EE IS EXPERIENCING LOWER BACK PAIN AND PAIN DOWN THE BACK OF THE LEFT LEG ALL THE WAY TO HIS FOOT. HE HAS BEEN SEEING A CHIROPRACTOR WHO HAS SUGGESTED THAT THIS MAY BE A CUMULATIVE TRAUMA FROM 31 YEARS OF BEING A LOADER OPERATOR. EE SAID HE HAS EXPERIENCED PAIN IN HIS BACK WHEN BEING JARRED DIGGING INTO A BLAST SITE.

EMPLOYEE WAS PULLING UP A LEVEL INDICATOR CORD FOR QUICKLIME #5 STORAGE TANK. THE CORD BECAME STUCK AND HE PULLED IT LOOSE AND FELT PAIN IN RIGHT SHOULDER AND NECK AREA.

THE EE (A LICENSED BLASTER) DETONATED A BLAST, WHEN A PIECE OF FLYROCK STRUCK HIM ON THE CHEEKBONE.

EE WAS SWITCHING GATES ON CONVEYOR TO SWITCH PRODUCT FROM #2 Q/L TANK TO THE QUICKLIME SILOS. EECLIMBED ON TOP OF CONVEYOR COVER TO SWITCH THE GATE. ON HIS WAY BACK DOWN HE SLIPPED STRIKING H THIGH ON THE HANDRAIL. THE CONVEYOR IS FOUR FT FROM THE WALKWAY AND THE HANDRAIL IS 42"" FROM THEWALKWAY. ON THE OPPOSITE SIDE OF THE GATE A LONGHANDLE WAS INSTALLED TO AVOID THIS FROM HAPPENIN

EE WAS OPERATING AN AUTOMATED PACKER WHEN A SPOUT BLOCKED UP. THERE IS AN EMERGENCY STOP THAT MUST BE ENGAGED BEFORE ENTERING THE ROBOTIC AREA. THE EMERGENCY STOP CUTS AND RELEASES BOTH THE ELECTRICAL ENERGY AND RELEASES AIR IN THE PNEUMATICS. EE DID NOT ENGAGE THE STOP COMPLETELY AND WAS STRUCK BY ROBOTIC ARM CAUSING INJURIES. SYSTEM OPERATING PROPERLY.

Flash burns (electric)

#7 DISCHARGE ELEVATOR ON THE OLD BALL MILL SYSTEM STOPPED AND WOULD NOT START. EE WAS CHECKING THE ELECTRICAL SYSTEM AND OPENED THE FUSE BOX FOR #7 DISCHARGE ELEVATOR. WHILE THE BOX WAS OPEN THERE WAS AN ELECTRICAL FLASH, SHORTING OUT THE ELECTRICAL SYSTEM. EE RECEIVED 1ST AND 2ND DEGREE BURNS TO BOTH OF HIS HANDS.

EMPLOYEE WAS PERFORMING SCHEDULED CLEAN UP UNDER A CONVEYER. HE WAS ON HIS KNEES REACHING MATERIAL WITH A SHOVEL WHEN HE EXPERIENCED PAIN IN HIS RIGHT SHOULDER AND NECK AREA. THE PAIN PERSISTE D AND HE WAS DIAGNOSED WITH A RIGHT SHOULDER/ CERVICAL SPRAIN.

LOADING COAL INTO COAL SYSTEM FOR THE KILNS. CHUCK OF COAL LODGED IN BOTTOM FEED OF BIN. USE A 10 TO 12 FOOT POKER, WEIGHTING 30 TO 40 POUNDS TO BREAK UP COAL. HAD TO BEND AT WAIST TO REACH CH UNK OF COAL. FELT PULL IN LOWER BACK.

ON MONDAY, NOV 12, 01 EMPLOYEE WAS WORKING ON A YARD VEHICLE. HE WAS ATTEMPTING TO REMOVE A PULLEY USING A WOOD WEDGE. HIS LEFT HAND SLID DOWN THE WEDGE CAUSING A WOOD SPLINTER TO LODGE IN HI S MIDDLE FINGER OF HIS LEFT HAND. HIS HAND WAS BOTHERING IN DECEMBER AND HE REPEATEDLY WENT TO THE DOCTOR TO SEE THE CAUSE OF A INFECTION. DOCTOR FOUND SOME SPLINTERS IN HIS FINGER.

EMPLOYEE WAS WORKING ON LOOSENING THE BRAKES ON A YARD TRUCK. HE WAS STRIKING A PIN WITH A HAMMER AND MISSED THE PIN HITTING HIS LEFT THUMB.

EE WAS USING AN AIR HAMMER TO REMOVE BRICK AND AN ASH RING FROM A KILN. HE FELT PAIN IN HIS LWOER BACK. HE CONTINUED TO WORK, A FEW DAYS LATER WE SENT HIM TO THE DR. BECAUSE HE WAS HAVING PAIN INHIS BUTTOCK AND NUMNESS RUNNING DOWN HIS LEGS. EE HAD AN ARI AND IS STILL WORKING BUT ON MEDICATION.

EE WAS OPERATING AN AUTOMATED BAGGING MACHINE. A BAG SLIPPED OFF THE SPOUT ADN EE REACHED INTO THE RESTRICTED AREA AND AGAINST SAFETY PROCEDURES AND WAS STRUCK BY THE ROBOTIC ARM - BRUISING HI S RIBS.

Fall to the walkway or working surface

EE WAS WALKING IN THE AREA OF #1 KILN STONE TANK. STEPPED ON 2"" LIMESTONE AND FELL ON RIGHT LEG, HURTING HIS RIGHT KNEE. EE WAS ON GROUND LEVE IN YARD WHEN HE FELL. NO DISTANCE IN FALL. FROM S TANDING POSITION TO GROUND BENEATH HIS FEET.

ON MAY 30, HE WAS CLEANING UP UNDER A CONVEYOR; HE ATTEMPTED TO LIFT THE WHEELBARROW AND FELT PAIN DOWN HIS LEFT LEG. ON JUNE 8, PAIN STILL PRESENT AND HE WENT TO DOCTOR.

EE WAS MOVING A LARGE PIPE AND AS HE WAS WALKING FELT PAIN IN RIGHT FOOT.

Over-exertion in pulling or pushing objects

INJURED AND ANOTHER EMPLOYEE WERE GUIDING A CATWLAK INTO POSITION. AS THE CRANE WAS LOWERING INTO POSITION, INJURED FELT A PULL IN HIS LOWER BACK ON THE LEFT SIDE. (THE CATWALK WAS ON THE JAW CRUSHER). THE PAIN INCREASED DURING THE DAY AND WENT TO THE DOCTOR IN THE PM.

Over-exertion NEC

REMOVING A COVER OFF OF #2 KILN COAL MILL WITH CO-WORKER. THE COVER WAS SLIPPING SO THEY LET IT DOWN TO THE FLOOR. WHILE INJURED WAS MOVING AWAY, HE SLIPPED CATCHING HIMSELF WITH HIS LEFT HAND . THIS CAUSED THE 3 MIDDLE FINGERS OF THE LEFT HAND TO BEND BACK.
