Struck by falling object

All we know is a call came 10/29/08 about 8:00 a.m. that we had a fatal accident at Kennecott; our driver was killed by a pipe falling off the top of load on trailer. All Bob Orton Trucking does is haul the load (this load from WL Plastics Corp, Cedar City, Utah). We do not load or unload product.

Employee was working down by ball mill 6 and 7 in the cyclone feed pump build up area. Employee was using the bolt for a hammer and smashed a finger.

Struck by flying object

Miner attempted to puncture aerosol can on vice at which time the can exploded, lacerating EE's face and chipping a tooth requiring stitches and outpatient cosmetic dentistry

Struck by flying object

While installing Tech screws into a panel, a metal shaving went in to employee's right eye. EE did not notice it until EE got in EE's car as was going home around 16:20. It felt itchy, and EE began rubbing it. EE didn't think much of it and EE thought EE would just flush it out when EE got home. The pain intensified, and EE called EE's supervisor to report it.

NEC

The death was caused by a heart attack, as confirmed on the death certificate. This was a personal medical event unrelated to work. No workplace factors contributed, and no controls could have prevented a condition not linked to occupational exposure.

Flash burns (electric)

Crush & Convey Electrical Team Member was called by the Crusher Control Operator to troubleshoot a power loss issue related to the magnet on the M3 belt. Employee began troubleshooting the 480V Breaker. During the troubleshooting process, an unexpected arc flash occurred which resulted in burn injuries to the top side of the Team Member's left hand and wrist.

Third party audiologist confirmed Noise Induced Hearing Loss of a Miner that is work related and greater than 25dB.

Fall onto or against objects

Worker slipped due to the muddy conditions and hit forehead on the end of a steel bar as EE fell. Deep laceration required stitches at a clinic.

Caught in, under or between a moving and a stationary object

Employee placed hand between tube and machinery while placing tube. Tube shifted causing finger to be caught between tube and equipment.

Struck by falling object

An Electrical technician was replacing a crusher mantle position probe, the probe came down pushing the technician's right arm downwards, contacting the elevated work platform top rail, and resulting in a fracture to the right ulna.

Over-exertion in lifting objects

EE was vacuuming a sump with an Air Mover Vacuum Truck & 6"" pipe and hose, when piping became stuck. After breaking the vacuum on the piping with the vacuum relief device which de-energized the vacuum to the piping, the employee injured back (back strain) while removing the stuck piping from the sump.

Struck by powered moving object

Snow covered concrete blankets were to be removed, the employee was hooking the rigging to the forklift when the bucket moved hitting them on the hard hat.

EE injured during a roll over vehicle incident. Driver stated EE saw something in the road and EE swerved. Pick up left the paved road and ended up in the ditch on the side of the road and rolled on to the driver side.

Struck by falling object

The employee was drilling two holes in a large back splash with a drill. The back splash was tied to the wall with rope. While drilling the holes, back splash fell on the employees left hand. The employee was wearing cut resistant gloves, however, the broken drill bit punctured the employees left thumb causing the laceration while pulling their hand out.

Over-exertion in pulling or pushing objects

While unhitching a light plant from a pickup. The employee stepped down onto the bumper to apply downward force and felt a pain in the right heel of their foot. Upon evaluation from a medical facility it was determined to be a distal Achilles heel sprain.

Employee removed cut resistant gloves to apply a clean label to a wire. Employee states they forgot to put gloves back on before moving to next task. While using a linemen's tool to score the outer jacket of the new cable the linemen's tool slipped and caught the tip of the left middle finger by the fingernail causing a laceration.

While unloading a pump the employee's hand slid across an exposed sharp edge on the tailgate of the truck resulting in a laceration to a finger which required sutures to close.

Employee's foot was stuck in mud / material, EE started to fall and let off the blaster hose that EE was spraying causing EE's momentum to go forward causing EE to fall.

Struck by falling object

Two Employees were working to remove a broken spreader beam from the body screen deck to install a new spreader. EE 1 was on the screen deck cutting the spreader to remove. EE asked EE 2 to come up on the crane after the spreader. When EE 2 came up on the crane the button on the controller got stuck on the up position, crane continued to go up breaking the beam free striking EE 1.

Employee was stripping a large cable with a razor blade. While stripping the cable, the razor slipped and struck EE's left hand resulting in a minor injury requiring two sutures. A commercial stripper was onsite but was missing a blade so the employee decided to use the razor blade instead and didn't exercise EE's stop work authority.

Struck by flying object

A mechanic was using a Porta Power to push a bearing on a drive pulley to achieve the final alignment. A bolt was used to extend the length of the porta power cylinder. The bolt ejected during the process and struck the mechanic in the chin.

Fall to the walkway or working surface

Employee tripped over edge of a piece of fire blanket that had been dislodged by another contractor and fell onto EE's knee. EE has been diagnosed with a hematoma and potential strain. No medical or work restrictions.

Struck by flying object

An employee was moving lifters to a cart with a forklift. EE did not lift the forks high enough which caused the lifter to fly up and hit another employee in the lip.

Fall from ladders

Employee was working inside the building on a sloped concrete floor. EE had tied the ladder to the structure and was in the process of welding a flange when the ladder slightly shifted startling the welder. EE then jumped down from the third rung of the ladder and landed on the concrete floor hurting EE's right heel.

Struck by flying object

Miner was working around an electrical box, when a dust particle entered left eye, on 7/24/19 it was not reported until next day 7/25/19. Miner was taken to clinic had a corneal abrasion requiring prescription antibiotics, Eye Drops. Returned to full duty no missed day

Over-exertion NEC

Employee closing a 24"" valve with a wheel handle, awkward access and hard to turn. Employee put their body weight into the valve handle to help get it to turn. When the valve handle finally loosened and was able to turn, the employee lost their balance and fell. In the process, they wrapped their arm around the valve to break their fall, tearing the tendon in the bicep.

Over-exertion NEC

Officer was hurrying to ensure there was no unauthorized access at the gate. Stepped off curb, twisted ankle. Step is 7 inches tall. Boots were laced, but loose. Officer has sprained ankle.

Employee was on a catwalk there was a slick spot of snow and ice, employee slipped fell and hit nose and lip on the rounded toe board requiring 4 sutures to close

During a sampling event at the Kennecott pit, employee lost their balance, fell down on right side/elbow while wading through snowy terrain resulting in a dislocated elbow.

Over-exertion in lifting objects

Craftsman was in the C9 trough replacing a return roller at the middle of the belt. EE had another craftsman on the outside edge helping remove and install. As the craftsman moved the roller into place EE felt a pop in the left shoulder and a sharp pain. Diagnosed on 6/14/2018 with full thickness tear to left shoulder.

Fall from machine

While preparing a reagent tanker for unloading an employee ascended the side ladder of the tanker trailer to access and inspect the vent cap. At some point, the employee lost balance and fell into the adjacent containment area onto a concrete surface.

Employee suffered a fatal cardiac arrest on 10/4/2017 due to a personal illness. Full description obtained by MSHA Inspector - Rocky Mountain District.

Unclassified, insufficient data

Employee visited a personal physician and described a rash on lower legs. The physician placed employee on restricted duty for two weeks. There are no details on if this is or is not work related, however, employee indicated to the personal physician it may be work related.

Over-exertion in lifting objects

While lifting a hydraulic wrench, employee strained right shoulder.

Struck against stationary object

An employee was in the process of completing a lockout when they dropped the lock on the grating, bent to retrieve the lock and when standing up, struck their right ear against a sink resulting in a laceration to the mid section of the right ear. The laceration resulted in 5 sutures placed on the outer edge of the ear.

Fall to the walkway or working surface

Employee indicated that while walking from light truck to a maintenance area employee slipped on ice and injured shoulder. Initially, and through today, this was felt to not be work related as the employee had other factors involved with the shoulder. This is continuing to be investigated through this date.

Caught in, under or between a moving and a stationary object

Employee smashed pinky finger of right hand between pry bar and steel structure holding the donut inlet tab above ball mill feed.

Accident type, without injuries

A lightening strike initiated a grass fire on the reclaimed south tailings impoundment. The fire was contained to approximately 5 acres, between 2 dirt roads. The fire was controlled while it burned itself out. No structures or people were at risk.

Struck against a moving object

A water truck became stalled and rolled back, contacting a pickup truck's front bumper. As a result of the impact, the driver's head contacted the dash/steering column. This resulted in a minor fracture to a lower back vertebrae. Employee was prescribed days away from work.

Over-exertion in lifting objects

On the date of the alleged incident, the illness was not reported. The employee complained of discomfort in back, due to lifting a jack out of a pickup truck, days later. Through medical evaluations and ultimately surgery on back to relieve pressure on a disc, it was determined to be a disorder due to repeat trauma and established as work related on July 12th

The employee was walking on grating at a pump station, employee slipped and fell on some ice. This resulted in employee striking right knee on the grating. Employee had knee swelling and complications weeks later......(Through medical evaluations, it was determined to be work related on 5/20/2016).

Over-exertion in pulling or pushing objects

Employee was opening a gate valve on water when he felt a pain in his left elbow. The pain did not completely go away and the employee was evaluated by a physician on 9/15/2015. The employee was diagnosed with a tear of the left bicep tendon. The condition will require surgical repair.

Absorption of radiations, caustics, toxic and noxious substances

An overcharged product delivery hose containing compressed air and lime pebbles separated from the connective fitting. The resulting blast of air and pebbles sprayed the driver's torso. The lime pebbles penetrated the driver's skin and burned him.

The death was caused by a personal medical condition. The employee was found unconscious and non-responsive. The incident is currently being investigated by MSHA as a non-chargeable incident.

The employee became confused and said that he wanted to find his father. He also claims to hear voices.

As a driver exited his vehicle, he slipped on ice, twisted his knee and fell hitting his back and head on the running board of the vehicle. After evaluation at a local hospital, he was diagnosed with a sprained/swollen knee and strained back.

Employee was tightening bussing from transformer to gear. Wrench slipped off striking #8 tooth. Employee went to see dentist. Dentist wanted to wait to see if pain subsided. No prescription meds, root & cap placed on Friday Dec. 5th became reportable.

Employee was assisting in moving 1/2"" steel plate when his right ring finger was pinched between the plate and his boot toe causing a crushing injury. X-Rays were positive for fracture of the end bone and sutures were required to close the wound.

Employee received laceration that required 8 sutures when his left ring finger was caught between bolt nut and pump flange. He was holding the nut with left hand while using an impact wrench to drive the bolt.

Over-exertion in lifting objects

A craftsman was assisting with the replacement of a conveyor belt scraper. He handed the new scraper through the access port of the head box to his co-worker & helped position it in place. As he did this he felt a pain in his abdominal region. Although the date of injury was 11/13/12 the employee was diagnosed with a hernia on 12/5/12.Employee is scheduled for surgery on 12/18/12.

An employee was in the process of repairing a trunnion grease line. The guarding was pulled away from the mill with a winch. Once the grease line was repaired, a wedge was placed in between and tension was released. The employee was then removing the wedge when it slipped out. The tip of his left middle finger was pinched resulting in a fracture and lacerations.

Employee was standing near forklift talking with the operator. The door was latched in the open position, a strong gust of wind unlodged the door and pushed the injured employee into the frame of the operators compartment from the back. His forehead struck the grab bar resulting in a laceration that required stitches despite the hard hat and safety glasses the worker was wearing.

Worker was cutting a section of toe plate with an angle grinder equipped with an abrasive cutting wheel when his right hand made contact with the cutting wheel, causing a 1cm laceration. Guard was in place. He was not using the detachable handle when the accident occurred. He had not received appropriate training prior to using the tool. The injury required 3 stitches.

Bolt in impact wrench came into contact with employee's mouth. Lip was lacerated, two teeth were broken.

Employee was replacing skirt liners in a conveyor transfer point. He was staging the liners by hand on top of the conveyor belt when his left little finger was pinched between two liners. The liners are approximately 40 pounds each.

Employee was working with another employee(Manlift operator) approximately 15 ' in the air performing structural steel bolt-up. He attempted to remove a beam clamp while the manbasket was still being positioned. This resulted in a pinch point injury between the beam clamp and the rail of the manlift.

I was starting down a 7% hill, when the lift truck popped out of gear, as the lift started picking up speed down the hill, my first instinct was to try to put the machine back into gear to regain braking control. The machine was a hydrostatic drive system. I was unable to put the machine into gear and decided to un-do my seat belt and jump from the lift.

Employee was cleaning particle size monitor intake hose screen. While doing this the screen contacted hand rail, causing the screen to bounce back and strike the employee in the chin. This resulted in a 2cm laceration on the chin. 4 sutures were required to close the wound.

EEs were conducting annual inspections of barges to check water levels & check for any corrosion. On last barge of the day, EE was grinding a bolt off in order to open barge lid when an undetermined pressure bent lid up & the pressure blew EE back. The pressure blew his safety shield & hard hat & caused laceration on forehead. The pressure burned his arms & small patch on neck.

A worn liner fell and struck another liner and a piece of steel flew off and struck an employee in the face.

The worker was in the process of picking up metal banding in the liner yard. The worker grabbed a piece of banding with both hands and tossed it to the side. As he did this, the edge of the banding contacted the top of his left hand, at the base of the thumb. This resulted in a laceration requiring 4 sutures to close. The worker was not wearing gloves as required.

The employee was raising Weld curtains when debris fell into the right eye; they immediately flushed the eye out; they went over to the Guard shack where EMT personnel are staged and they also flushed the eye; the employee was then taken to the nearest Medical facility; they gave him an antibiotic eye drop solution to prevent infection and released him back to Regular duty.

Operator was position guard, felt slight pain in shoulder resulting in inflamed rotator cuff - Restricted duty

OPERATOR WAS MOVING A WASH WATER HOSE WHEN THE NOZZLE IN HIS RIGHT HAND CAME IN CONTACT WITH HIS FACE, CAUSING A LACERATION TO THE UPPER LIP REQUIRING 6 SUTURES. THE HOSE WAS NOT ENERGIZED AT THE TIME OF THE INCIDENT.

Flash burns (welding)

Employee experienced burns to his eyes resulting from exposure to welding arc while welding and helping with welding activates. The employee was issued eye drops and pain medication to treat his condition. The employee has fully recovered from the incident.

An operator was responding to an alarm on the M3 belt when he slipped on 3/4 inch nut that was left on the diamond plated metal floor. The force of the fall broke the team member's hip.

EE was working on a pump, he slipped and fell striking his head on a piece of conduit.

Struck by falling object

A razor sharp edge of the liner cut through Troy""s leather glove when he dropped the liner segment. As the piece fell it lacerated 3 fingers on his left hand. The PPE was not appropriate to protect him from the sharp edge of the worn liner. He failed to recognise the hazard and take appropriate action. The liners were worn beyond reline limit to failure.

EE was on corrugated roof and tripped, falling backwards onto hind end, cutting palm of right hand through glove on a piece of aluminum insulation wrapping. Cut was minor, but required stitches to close. EE returned to work same shift with no restrictions.

Crew engaged in line cleaning operation. Pressure dump device was operated by co-worker (procedure violation). Work activity was suspended to verify process issue, EE placed L hand over nozzle to block low pressure water bleeding from nozzle. Co-worker asked if he was ready & mistook no for go. High pressure water resulted in laceration. Surgery initially proposed & declined.

Using 1"" air impact wrench to intstall bolts on pipe flange. Socket slipped off bolt and smashed index finger between bottom of impact gun and flange, causing laceration that required sutures.

Employee slipped on snow/ice while retrieving lumber from decant box and struck chin on grating causing laceration that required two sutures to close.

While assisting with the removal of a motor, ee reached in to pull one of four bolts out of the motor base, grabbing the bolt head. While he had hold of the bolt, the motor shifted slightly, causing movement of the bolt head, pinching the end of the employee?s right middle finger between the bolt head and the motor housing causing a

Employee was removing old fire suppression piping from cooling tower #1 at Kennecott Utah Copper Power Plant. The pipe bounced when coworker pulled on it striking employee above left eye. The result was a laceration requiring 5 stitches.

Employee was tearing down a scaffolding and an aluminum plank was moved which jammed employee's finger between the plank and a metal structure, resulting in a smashed left index finger. this required 6 sutures anda tetnus shot.

While replacing the lower section of a Sag mill motor bearing, an eye bolt was attached to the section and an overhead crane was being used to move/position the bearing section when the eye bolt broke and grazed the craftsman's left arm above the wrist resulting in a laceration which required 4 sutures. The employee was wearing gloves.

Employee slipped on the edge of a landing at the top of a set of four stairs. He fell backwards and his elbow struck the deck of the grating on the same level. He sustained a small laceration to his left elbow, which required three sutures.

Employee was bending over trying to roll some pipe to read the line number and later noticed some back pain.

As employee finished grinding steel pipe, he raised his face shield, allowing a metal flake to fall into his (R) eye. A metal foreign body & rust ring were removed by medical provider. Employee was treated with presc. medication & released to return to regular duty with no restrictions.

Fall down stairs

While walking down the stairs that were positioned in the crusher pocket during maintenance activities, the worker caught his foot on the bottom stair. This caused the worker to fall and hit the back of his head on the staircase. This incident requred six sutures to the back of the worker's head.

After completing task, employee lowered the JLG basket to the ground. As he exited the basket, he stumbled and fell on a nearby wooden crate. Employee received a laceration to his right forearm which required eight sutures.

Over-exertion in lifting objects

Employee was installing fiberglass insulation in the spare crusher bow assebly. He injured his shoulder as he lifted himself out of the bow assembly. The height is approx. 4.5 ft As he lifted himself out, his right shoulder gave.

The ee was in the process of removing a sump pump with an overhead crane. He positioned his hand on top of the pump motor in efforts to help guide the pump while raising. During this time ee's left little finger was pinched between the pump motor and a piece of angle. iron that was attached to the wall.

The EE was in the process of removing a tail pulley bearing race. He was unsuccessful in removing the race by grinding it down, so he struck it with a hammer. A fragment shot off and struck him in the left leg. The EE and supervisor inspected his leg and found no injury. On 1/28/07 EE felt discomfort in his leg, went to instacare, X-rays showed metal fragment and it was removed.

Employee was changing liners in a ball mill feed cart using a pry bar. The panel slipped into position and the employee smashed the end of his left ring finger between the pry bar and another liner panel.

The EE passed away while working along the conveyor belt below the crusher. The worker was positioned on the catwalk/walkway, when he lost consciousness. A co-worker caught him and laid him on the walkway. The death is believed to be caused by natural causes.

Incident occurred at the Utah Power Plant where the employee was slagging the #2 Boiler. Employee had improper hand placement on the slagging hook and pinched his left ring fingertip between bar and frame.

Employee sustained a cut on his left wrist while installing a piece of ducting. The cut resulted in eight stitches.

Employee stepped on a piece of channel iron, lost his balance and fell onto the channel. EE is on light duty.

Struck by falling object

Employee was rolling over concrete form approx. 2'x6' on the ground by placing metal stake on top of form. The stake slipped causing the form to strike and slide down front of employee's left leg (shin) causing contusion with no broken skin. On 5/13/06 the wound became infected and treatment for infection began.

EE was setting equipment into place by hand. Power distribution unit overturned and hit him in the face.

Employee was in the process of checking the current draw across an electrical breaker. During the process, the power breaker flashed causing burns to his face and eyes.

Contact with electrical current

A mobile crane was moving a metal shed. An employee was on the ground guiding the shed. The EE on the ground was contacting the metal shed when the crane came into contact with a 13.8 KV powerline. The EE on the ground received an electric shock and fell to the ground. This caused an entrance wound to his hand and exit wound on toe.

Employee was standing on a trailer bed and the boomer was waist high. He set the truckers bar on the boomer and released the boomer. It swung across, striking him across the right eyebrow.

Fall from machine

Tasks related to a valve switchover. Employee just completed hosing off floor. Employee was sitting on a 14"" steel pipe (approx. 48"" high from concrete slab) & went to stand on top of pipe. Employee lost his footing & fell to concrete slab. Fall resulted in a fractured wrist. Further testing is being conducted on a potential herniated disc in the lumbar region.

Employee was working on the tailings pipeline overflow during the timeframe of the incident. Employee does not know if the wound/infection was caused by a splinter or insect bite.

Utah Power Plant employee was showering after his shift was completed. While washing his face, employee got soap in his eyes and they started to burn. Employee notified the plant emergency response team. The employee complained of a 'foggy' haze and extreme pain. Employee was taken to the emergency room and the doctor gave an antibiotic as a precautionary treatment.

EMPLOYEE WAS MOVING EMPTY BAGS AND FELT A SHARP PAIN FROM AN INSECT BITE.

Employee was pushing a wheelbarrow up a ramp to dump into the bottom ash silo. While ascending the ramp, the axle slid out of the bearing, the wheel barrow stopped suddenly, and the employee jammed his arm/elbow. This action caused a hairline fracture to the employees left radial head (elbow).

Man was trying to get a not out the socket on an impact wrnech. Wrench had airline connected. Man's right hand contacted trigger on wrench and spun the not up to the 2rd joint of his left index finger.

Absorption of radiations, caustics, toxic and noxious substances

Employee was unbolting a flange on a caustic pipeline when solution splashed his forehead and trickled into both eyes. Employee neglected use of proper PPE (splash goggles & faceshield) for this task. Eyes were washed out and employee was seen at the company clinic for evaluation. Doctor gave employee an antibiotic to prevent infection and released to full duty.

WHILE CUTTING A PIECE OF PLASTIC WITH A BAND SAW A PIECE GOT STUCK. WHEN THE EE ATTEMPTED TO FREE IT, EE HAND BOUNCED INTO THE BLADE.
