Caught in, under or between a moving and a stationary object

IW & coworker were installing a roller and IW pinky got caught underneath, causing a laceration and fracture.

Caught in, under or between a moving and a stationary object

IW approached a small crane that was beginning operations. EE failed to alert the operator of EE's whereabouts. EE then stood in a blind spot for the operator and as the outriggers were being lowered EE got EE's foot caught.

Struck against stationary object

IW was mounting haul truck after break. EE was not paying attention, EE was looking at EE's phone. EE bumped EE's head on the overhang of the truck bed.

Struck by rolling or sliding object

Employee said that they were working down in the hole, there was a roll of plastic liner on a rack being. As they were rolling out the plastic liner, the plastic liner slid down the rack and crushed employee's left hand between the liner and the rack.

Struck against stationary object

Entering haul truck 06-0034 turned to open door, hit head on ROP guard (height of door 62.5 inches).

Fall from machine

Servicing 26-0073 from top step, right side rear step 35"" from floor changing filters, foot slipped off dirt on step, employee fell to shop floor.

Fall from machine

Climbing off screen equipment, fell on the ground (4ft approx.). Left ankle sprain, facial contusions.

Over-exertion in pulling or pushing objects

Employee said he was attempting to roll a rock off to the side of the haul road.
