Struck by flying object

Employee was hit with a clevis in the mouth resulting in a busted lip and broken teeth.

Employee was helping other employees carry a metal plate and they dropped it on their finger, fracturing it.

Trying to pry a pulley off, bar slipped and caught their thumb between the two.

EE was shoveling in between the shuttle conveyors of the bottom floor of the Mill. EE was shoveling bent over to avoid the conveyor above EE and twisted after getting a shovel full to throw into the bin. EE then felt a shock in the lower back.

Either a hose or cooler burst (unknown at this time) spraying the turbo with compressor oil leading to a fire on the rig (extinguished in under 30 minutes). Driller leapt from the rig breaking their left heel, spraining right ankle, and second degree burns on left forearm and right hand (grabbed the handrail to leap over). Accident is investigation underway this week.

Employee was trying to remove the lock pin on the #4 crusher with a pry bar and dropped the bowl on their middle finger.

Employee was moving a cross member and dropped it, smashing EE's R ring and middle finger.

EE and another employee were moving a motor over to a conveyor. When they reached the pedestal, EE had EE's hand underneath and it got cut.

Employee was helping other employees change out a conveyor when a rock that was lodged in a roller broke loose and shot out at the employee, hitting EE in the chin. EE received 6 stitches and 1 day lost time.

Employee was trying to remove a tooth from a loader bucket with a hammer and part of the tooth chipped and entered EE's abdomen.

Employee had called in in April with a personal back injury and has been treated by PCP. EE has been on FMLA for several months and now wants to say this is work related. We are denying this claim.

Employee felt pain in shoulder after pulling down on a conveyor belt. EE did not seek treatment until 7/9/18 and did not have any restrictions. Was seen again on 7/11/18 and now has restrictions.

Employee was trying to free up a gate under a bin and finger was smashed in the gate.

Employee fell into the discharge chute and hit head, requiring stitches.

Employee was loading rock into a basket when one rolled back and smashed left thumb.

Employee was swinging a sledge hammer when employee felt a pain in employee's back. Employee finished employee's day and when employee came back to work today, employee wanted to go to the clinic.

Employee was coming down out of the haul truck when EE fell on EE's behind and had pain in the back.

Employee was driving a haul truck down a haul road and rolled the truck over.

Employee was working Saturday and Sunday driving on rough terrain when employee's back started hurting. Employee was off on Monday and when employee came in on Tuesday, employee hit a bump in employee's haul truck and hurt employee's back again.

Employee was installing a motor when the wrench he was using slipped causing him to lose his balance and fall backwards hitting his head. He also complained of chest and back pain.

EMPLOYEE IS UNABLE TO RECALL THE FALL OR ANY CONDITIONS LEADING UP TO THE FALL. EMPLOYEE HAS 5 FRACTURED RIBS, BOTH LUNGS WERE PUNCTURED, 4 FRACTURED VERTEBRAE, AND A SPINAL CORD INJURY.

Employee had been operating the stockpile loader 5/7/12 and 5/8/12. He called in the morning of 5/9/12 that his back was hurting and needed to go to the doctor. He has a low back strain.

At 2:15 pm employee stated that while operating loader he had felt a pain starting in his ribs and working its way to his groin.

Employee was using a prybar to lift roller for repair when the pry bar slipped causing the employee to fall.

First reportable treatment date is 4/25/12 in which he received an injection for a knee strain he did when he stepped in a shallow hole and twisted his knee. He hasn't had anything other than ""first aid"" until now.

He was cleaning the mirror with his hand on the handrail on the Cat truck. The handrail came loose and he fell to the ground. No medical treatment at that time. He decided for medical treatment on 5/20/12 and was released from care with the understanding that if he cont'd to have problems they would need to scope his knee. He had surgery 11/1/12.

Employee was walking down the stairs from the Control Room and missed the bottom step, falling and twisting his ankle.

Employee was cleaning sludge from a raised drain pan when the metal lifting strap on the side of the pan broke off and struck him above the left eye.

Employee was climbing over a discharge belt and grabbed a V-belt. The weight caused the belt to move, his left hand got stuck in the belt and pullet, cutting off the tip.

EE allegedly strained his lower back while pulling on a trailer that was stuck, to get it released. EE did not report accident to the company until 9/15/10 and did not seek medical attention until 9/13/10.

EE was using a pry bar to dislodge clay and rock from crusher slide. The material broke off, and caused the pry bar to kick up and hit him in the face.

Cleaning the window on his truck, had his right hand on the ROPS when the wind blew the door closed on his hand.

EE was welding to install a screen in the CR plant. He felt pain in his left hip. He did not seek medical attention and begin losing time until 16 June 2010.

EE was cutting loose the feedbox on the screen unit. The box flipped when lifted by crane and smashed leg against screen unit.

Injury was using a hammer wrench. A nut came loose and he smashed his finger between the wrench and a speed reducer.

He was removing air tank from truck and slipped, injuring his knee. He did not begin working restricted duty until 1 June 2010.

Employee was cutting a section out of the floor in the mill. He stepped on a piece that he had already cut and fell through the floor.

Injured was using a sledge hammer to close a bin gate. His finger was smashed between the hammer and a metal beam.

EE was cleaning under a conveyor belt. He raised up and heard a pop in his back and felt sharp pain.

Injured EE was using the manlift to service a hoist. He let the boom down on a piece of pipe, lifting a wheel off the ground. He fell from the manlift bucket but was caught by his fall protection.

EE was dumping a load of rock over a handrail with a skid-steer loader. The loader tipped forward. EE reached out to try to push away from handrail. His left hand was caught between the handrail and the bucket.

Injury was driving backhoe to Delta Wing, a gust of wind blew his safety glasses off and dirt got in his right eye.

Injury was walking under a belt. He stood up and walked into a second conveyor, hitting his face on the metal frame. The bridge of his nose was injured.

Employee was setting brakes on railcar. When he climbed down, he missed a step and fell. He injured his hand, elbow and shoulder.

EE's arm was hurting from shoveling on the 8th of Dec, then he used a sledge hammer on 10 Dec and hurt it again. He did not seek medical attention until 12/15/08 and began working restricted duty 12-15-08.

Employee was walking from the mill clockhouse to the parking lot and was struck from behind by a fellow employee in his personal vehicle. He fell forward and cut his forehead and knee. His only injuries are scrapes and bruises.

EE was looking in the bins on shuttlefloor. He turned around to go back to the control room and struck his head/hard hat on a piece of angle iron on the gathering conveyor. His forehead above eye was cut and he suffered a mild concussion. Bruised left arm and side.

EE was helping to move a shaft with a strap. The strap slipped and the shaft fell and struck the top of his foot.

EE was trying to insert shims under the scrubber jack while another employee held it up with pry bar. The bar slipped and smashed both of his index fingers. No lost time but stitches were required.

EE was grinding a piece of 1-inch angle, when his hand was pulled into wheel on bench grinder. He suffered a left hand laceration.

EE was holding a bearing puller in place while another employee was adjusting a plate with a hammer. The other employee missed and hit injured EE's finger with hammer. No lost time, no light duty, but finger was fractured.

Employee was making a hose for a loader and smashed the ring finger on his left hand. The tip of his finger was amputated.

Employee was changing a unit and cut his hand on a piece of metal on the side of the dust cover. There was no lost time, no light duty but stitches were required.

EE was walking down the steps on the mill and slipped on ice and fell 5 or 6 steps. His back was injured. He is working light duty.

EE was walking down stairs at the Phase 1 building when she fell. She did not seek medical attention until April 2007. She was given restricted duty beginning 05/05/07.

Employee mashed the ring finger on his left hand between the handle and a concrete bucket while pouring concrete out of a bucket. His finger was lacerated. He is working restricted duty.

Employee was unhooking a strap from a crane. He lost his footing and fell off a rockbox at the tail of a conveyor. He landed on the ground and broke the wrist on his left and the upper part of his arm on the right side. He is losing time.

Employee was working with another employee to remove a block bearing from a shaft when the hammer slipped out of the other employee's hand and struck injured in the face. His nose was fractured and he is losing time.

Employee was on the catwalk at the wash plant. Instead of using the ladder, he jumped from the catwalk to the ground. He landed on a chips pile and strained his right ankle.

EE was working on the feeder. When he stepped away from the feeder, his ankle twisted. His right ankle is sprained. He is working restricted duty.

Employee was using his pocket knife to remove an old O-ring from a hose when the blade lock failed. The blade folded over and cut the middle finger on his right hand. There was no lost time and no restricted duty but three sutures were required.

EE was servicing the portable impactor. While he was carrying the waste oil down, his foot slipped and he sprained his right knee. He is on restricted duty.

Employee was getting a sample off the conveyor belt when he slipped and struck his left side on a brace. His ribs were bruised.

EE was climbing down a ladder on an IT28 (loader) and strained his right ankle.

Employee was helping other employees rotate the tire on a Caterpillar 992D loader so they could drain the oil. He felt pain in his back. He sought medical treatment and began working restricted duty on 8/5/05. He returned to full duty 8/12/05.

Employee was cleaning the slide on a tail pulley when the take up slid forward and smashed his left hand. His left finger was cut. EE missed three days of working and is now working restricted duty.

Employee developed lower back pain while loading stockpile trucks at the bins. He sought medical attention on 13 June 2005 and missed one day of work.

Struck by powered moving object

EE was walking under the bins & stepped in front of a stockpile truck. The driver of the truck attempted to stop but could not & struck EE. He suffered bruises/scrapes to his left arm/left hip. The accident happened 2:30 am 04/14/05. He missed the night shifts of 04/14, 04/15, 04/16, & 04/17. He returned to work full duty on the evening of 04/18/05

EE was dumping a wheelbarrow of rock that he had loaded. He strained his groin and stomach. No hernia.

EE was shoveling on 15 November 2004 and reported that he pulled a groin muscle. He did not seek medical attention until 19 February 2005 and began losing time 19 February 2005.

Employee was working with another employee to do a kill cable check. He leaned over to see how much rock was left on the screen. When he did, he placed his hand on the motor stand. The other employee turned off the motor and it came down on employee's right index finger. His finger was mashed.

Employee was carrying rock samples from the mill. He missed a step and fell, straining his neck and legs. He began losing time 10/19/04.

Employee was using a sledgehammer to put a timing gear on the shaft on a screen unit. The thumb on his right hand was smashed between the unit and the handle of the hammer. He began working restricted duty on 13 October 2004.

Employee was lifting his toolbox out of his locker and felt a sharp pain in his right elbow. His elbow was strained. He is working restricted duty.

Employee was cleaning railcars and started to slip. He tried to grab a bar and caught his left hand. He sprained his fingers and wrist on his left hand. There were two days of lost time and EE is still working restricted duty.

EE was working with another employee on a Caterpillar truck. The wrong hose had been used to hook up the 3/4"" impactor and when the air was turned on, the hose came loose and swung back and struck EE'S forehead and lip. There was no lost time, but stitches were required.

EE WAS CUTTING A PIECE OF 2""AIR HOSE WITH A DIE GRINDER AND A CUTOFF WHEEL. WHILE CUTTING THE HOSE THE DIE GRINDER SLIPPED OFF OF THE HOSE WITH THE CUTOFF WHEEL CUTTING HIS LEFT HAND. CUT REQUIRED STITCHES.

Employee was cutting a bolt on a belt puller. A hot piece of metal fell on his pant leg, burned through his pants, fell inside his boot and burned his right foot. Employee is off work beginning 4/25/04.

Rubbed or abraded

Employee's right knee was bruised while he was kneeling down working on a Caterpillar 773D quarry truck. Employee did not seek medical attention until 19 April 2004. He missed one day of work.

Two employees were in a manlift at the construction site. The lift was inadvertently raised into a beam. One employee was caught between the manbasket and a metal frame. His chin was cut and a vertabra in his neck/back was injured. He is losing time.

EE got out of a pickup truck and set the parking brake. When he exited the truck, the parking brake failed. The truck rolled backward and ran over EE'S leg. His leg was bruised. He did not seek medical attention until 12 April 2004 at which time it was discovered that his leg was fractured. EE is losing time as of 12 April 2004.

EE WAS CUTTING A PIECE OF METAL AT THE CONSTRUCTION SITE. A PIECE OF HOT SLAG FELL ONTO HIS FOOT AND BURNED IT. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 21 DECEMBER 2003.

EE WAS ON THE CATWALK UNDER THE MILL. TRUCK DRIVER WAS UNDER THE BINS IN A HAUL TRUCK. EE HAD HIS PAYCHECK SO HE CLIMBED OVER THE HANDRAIL INTO THE HAUL TRUCK'S BED TO HAND THE DRIVER HIS CHEC K THROUGH THE WINDOW. THE DRIVER NEVER SAW HIM & STARTED TO DRIVE OFF.
