EMPLOYEE FELT SICK AT THE END OF EE'S SHIFT. EE WENT TO THE ER AND FOUND OUT EE HAD BRONCHITIS. EE RECEIVED MEDICATION.

Walking on walkway at loadout area and tripped on a piece of steel sticking out of walkway.

EMPLOYEE WAS CLEANING UNDER 1010 BELT LINE WHEN EE WENT TO STEP OVER A ROCK AND TWISTED EE'S RIGHT KNEE.

EMPLOYEE WAS EXITING OUT OF SURFACE RECLAIM TUNNEL, BETWEEN THE STAIRS AND EXIT, TURNED RIGHT AND FELT PAIN ON THE INSIDE OF THE LEFT KNEE. THIS HAPPENED ON 05/19/2025. EE WAS NOT PLACED ON LIGHT DUTY. EE HAD SURGERY ON 08/12/2025. EE IS NOW LIGHT DUTY.

DEBRIS FELL BETWEEN GLASSES AND EMPLOYEE'S RIGHT EYE.

WHILE OPERATING THE TRAIN, THE EMPLOYEE THOUGHT THE LOAD FELT LIGHT, STOPPED, PUT ON THE PARK BRAKE, AND WAS GOING TO SEE WHAT THE ISSUE WAS. BEFORE EXITING THE TRAIN, TRAIN CARS THAT CAME OFF OF THE TRAIN HIT THE BACK CAUSING THE EMPLOYEE TO FALL FORWARD IN THE TRAIN AND HIT LEFT LEG AND LEFT CHEEKBONE.

EMPLOYEE WAS SWATTING AT A WASP AND WAS STUNG ON RIGHT-HAND PINKY FINGER.

Employee was working in J21 and located in an Elliot basket boom that was in an elevated position. They were using a slate bar to scale down loose material on the rib. As they were prying loose a rock on the rib, they felt pain in their abdominal area and then continued working the remainder of the shift.

EMPLOYEE REPORTED ISSUE ON 07/15/2024 BUT DID NOT GO TO THE DOCTOR. EMPLOYEE THEN CONTACTED REPRESENTATIVE AT DOE RUN ON 09/23/2024 ABOUT HAVING ISSUES. EMPLOYEE DID NOT GO ON LIGHT DUTY UNTIL AFTER EE'S SURGERY ON 06/04/2025. EMPLOYEE STATES EE WAS MUCKING AND HAULING OUT OF A ROUGH AREA IN THE MINE. AFTER THIS THE EMPLOYEE STARTED HAVING LOWER BACK ISSUES.

Employee was operating a stand up ditch witch skid steer. The stand-up ditch witch caught a hose on the wall in the tunnel on the back door cover plate on the hydraulic side, which pulled the machine sideways and propped the door like it was running over a rock, and when it did it caught the right thumb on the hand rest, there for partially amputating the right thumb

A rock rolled into the walkway of the dump operator's shack. Injured employee moved rock (per the accident report the size of a volleyball) out of walkway and felt pain in lower back.

Employee was feeling underneath engine for hose and cut their right thumb on the fan shroud.

EE WALKING AROUND POWDER TRUCK AND HIT RIGHT KNEE ON A PIECE OF ANGLE IRON RESULTING IN LACERATION REQUIRING SUTURES. EE WAS RELEASED BACK TO FULL DUTY FOLLOWING SUTURES.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME / CUBITAL TUNNEL SYNDROME ON 5/9/2023. TREATMENT AND/OR SURGERY IS PENDING AT THIS TIME.

EE CUTTING CHAIN WITH HAND GRINDER. GRINDER BOUND UP AND SLIPPED AND EE GOT A CUT ON LEFT HAND BETWEEN THUMB AND INDEX FINDER REQUIRING 5 SUTURES.

EE STEPPING OFF DRILL TO CHANGE A DRILL BIT, STEPPED ON ROCK, RIGHT KNEE POPPED AND FELT PAIN. MEDICAL EVALUATION ON 03.06.2023 EE WAS PLACED ON RESTRICTED DUTY AND REMAINS ON RESTRICTED DUTY AT THIS TIME.

IMPACT AND SOCKET CAME OFF BOLT WHILE RE-TORQUING BOLTS. EE FELL LANDING ON PINKY FINGER OF LEFT HAND. EE RECEIVED SUTURES IN FINGER AND PLACED ON RESTRICTED DUTY AT THAT TIME.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL / CUBITAL TUNNEL SYNDROME ON 01/16/2023. TREATMENT AND/OR SURGERY IS PENDING AT THIS TIME.

Individual was walking around the rod table to turn off a generator. When walking around the end of the rod table they stepped on some uneven gravel and is when they rolled their right ankle.

EE getting steel off rack and caught foot on rack twisting left knee. EE did not seek medical treatment until 10/10/2022 and was then placed on restricted duty following further testing.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME ON 8.24.2022. TREATMENT/SX PENDING.

EE HIT ELBOW WITH DOOR WHILE EXITING ROOF BOLTER. EE PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE DIAGNOSED WITH BILATERAL CARPAL / CUBITAL TUNNEL SYNDROME ON 7/26/2022. SURGERY DATES ARE PENDING AT THIS TIME. FOLLOWING SURGERY EE WILL BE RELEASED TO RESTRICTED DUTY.

EE WAS LOOSENING A BOLT TO CHANGE THE OIL ON A DRILL WHEN THE EE FELT PAIN IN UPPER RIGHT ARM / SHOULDER. EE WAS EVALUATED BY MEDICAL PERSONNEL AND RELEASED BACK TO NORMAL DUTIES. EE HAS RECENTLY BEEN RE-EVALUATED AND TESTING PERFORMED RESULTING IN A SURGICAL RECOMMENDATION FOR A TORN ROTATOR CUFF. SURGICAL DATE IS PENDING AT THIS TIME. EE REMAINS FULL DUTY UNTIL SURGERY DATE.

EE DIAGNOSED WITH BILATEL CARPAL TUNNEL SYNDROME. TREATMENT / SURGERY IS PENDING AT THIS TIME.

EE diagnosed with occupational carpal tunnel syndrome. Surgery is recommended. EE will be released to restricted duty following surgery.

Apx. 760' HPDE pipe was being pulled down an access road. Around a 100 degree bend, the pipe developed resistance from unidentified debris (presumed rock). The resistance gave way and the tension applied from pulling the pipe caused it to straighten striking and employee inspecting for holes.

EE CHANGING BOOM ROLLERS, SLIPPED ON THE BOOM ROLLER COVER AND FELL ON LEFT SHOULDER AND RIGHT KNEE. EE PLACED ON RESTRICTED DUTY ON 8/16/2021

EE twisted left knee stepping on a loose rock on the ground. EE placed on restricted duty on 07/12/2021 after requesting a medical evaluation.

EE MOVING BATTERIES ON A PALLET AND FELT PAIN IN LEFT SHOULDER. EE SOUGHT MEDICAL TREATMENT, TESTING SHOWED TEAR IN ROTATOR CUFF THAT WILL REQUIRE SX. EE REMAINS ON FULL DUTY AT THIS TIME. SX SCHEDULED 4.21.21 AND WILL BE RELEASED TO LIGHT DUTY FOLLOWING SX.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME ON 01/15/2021. SX PENDING AT THIS TIME. FOLLOW SX, EE WILL BE RELEASED TO RESTRICTED DUTY. EE REMAINS FULL DUTY UNTIL SX.

EE stepped on ballast rock and felt pain in left foot. EE placed on restricted duty on 9/22/2020 for pain and further medical evaluation.

EE TWISTED RIGHT KNEE WHILE EXITING DRILL. EE PLACED ON RESTRICTED DUTY.

EE felt pain in back while sitting down in loader seat. EE taken for medical treatment and released with restrictions. Medical treatment is ongoing at this time.

EE going underground on cage and felt ear pop due to elevation change. EE remained full duty until surgery on 9/22/2020 and was placed off work at that time.

EE grabbed another EE that lost balance to keep EE from falling. EE felt pain in right bicep. EE placed on restricted duty pending testing.

EE diagnosed with Carpal Tunnel Syndrome and Cubital Tunnel Syndrome of the left arm on 6/10/2020. EE remains full duty until surgery (scheduled for 7/9/2020) and will be released to restricted duty following surgery.

EE flushing out a ZnSo4 line with an air hose, the hose came loose and hit employee in the face. EE received a scratch on right eye. Antibiotic eye drops were prescribed to employee.

EE untangling power cable and felt pain in right shoulder. EE did not seek medical attention at the time. EE requested medical attention on 5/12/2020 and subsequently placed on restricted duty at that time. EE remains on restricted duty pending medical treatment.

EE STEPPED ON ROCK WHILE WALKING AND TWISTED RIGHT ANKLE. EE RECIEVED NO MEDICAL TREATMENT AT THAT TIME. EE REQUESTED MEDICAL TREATMENT AND WAS DEEMED WORK RELATED PER EMAIL ON 4.27.2020. EE WAS PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE operating a walk behind loader. EE slipped off platform, while slipping off EE grabbed control and moved loader backwards pinning lower leg between loader and concrete. EE placed on restricted duty on 7/8/2019.

EE lifting 50lb bags of anfo to dump into charging rig hopper and felt pain in neck and shoulder. EE received medical treatment on 7/08/2019 and was placed on restricted duty at that time.

EE diagnosed with occupational carpal tunnel syndrome on 2.21.19. Treatment and/or surgery is pending at this time.

EE twisted right knee when exiting piece of equipment on 11/26/2018. EE remained on full duty following medical evaluation and testing. EE will receive surgical treatment on 5/24/2019 and will be released to restricted duty on 5/25/2019.

EE stepping onto train and felt pain in left knee. EE remains full duty at this time but will have surgical ""scope"" procedure on 2.1.19 and will be placed on restricted duty following procedure.

EE smashed thumb between steering cylinder and frame resulting in small chip fracture of left thumb. EE was released to full duty following medical evalulation.

EE lifting a piece of plate steel and felt pain in right elbow. EE received medical treatment and was placed on restricted duty.

Possible unconfirmed insect bite/sting. EE working on a pump and noticed a small blister spot on side of right hand. Hand/arm began swelling. EE treated at local clinic but could not confirm diagnosis of insect bite sting. EE released to Full Duty.

EE straining while lifting liners and felt pain in groin area. EE diagnosed with hernia. EE placed on restricted duty while treatment is pending.

EE was loading glue on bolter when EE hip popped out of place and back into place pinching nerve. EE was released to full duty following medical examination on 4.10.18. A follow up appointment on 5.1.18 resulted in EE placed on restricted duty at that time.

EE diagnosed with occupational carpal tunnel syndrome on 8.20.18. Treatment and/or surgery is pending at this time.

EE climbing up steps to get into loader when EE twisted left knee. EE placed on restricted duty following medical treatment.

EE received a laceration on left forearm when the utility knife slipped while EE was cutting a zip tie. EE received sutures and was released back to work.

EE diagnosed with Carpal Tunnel Syndrome on 5/4/2018. EE remains full duty pending treatment/surgery. If EE has surgery, EE will be released to restricted duty following surgery. Treatment and/or surgery is pending at this time.

EE performing hand scaling underground with a scaling bar and felt pain in right elbow. EE was sent for medical treatment on 3/14/18 and was placed on restricted duty at that time for elbow sprain.

EE moving a steel box when the plate clamp slipped and the steel box fell onto EE right foot fracturing a bone in right foot. EE released with light duty restrictions following ortho specialist exam.

EE operating haul truck underground, bed raised while driving and stuck in back causing EE to hit steering wheel. EE was taken for medical treatment and cleared for light duty until seen by company doctor. EE was seen at company doctor on the first shift back, 11/13/17 and released to full duty.

EE lifting water lines and felt pain in abdominal area. EE taken to local clinic and sent to ER for diagnosis and treatment. EE then transferred to Mercy Hospital in Festus Mo for surgery of hernia.

EE felt pain in low back when lifting a 40 lb box of resin onto the roof bolter. EE went to the doctor on 8.14.17 and was placed on restricted duty at that time.

EE hand scaling and felt pain in low back and right shoulder. EE placed on restricted duty following medical examination.

EE attempting to clean at #5 crusher train dump when EE lost footing and fell into open hole. EE received sutures in a laceration on bottom lip and was placed on restricted duty for knee pain.

EE was pulling on a wrench to loosen a cartridge when the cartridge broke loose causing EE to hit elbow on the fender. EE was placed on restricted duty on 6/14/2017.

EE removing rock from under coarse ore bin when rock came in cab and hit ee on the right arm and hand. EE received laceration and sutures and placed on restricted duty.

#2 Service hoist went down at 2:30 am on 8/27/16 due to a burnt coil. Electrical was called and coil was replaced. Hoist was placed back into service at 4:47 am on 8/27/16.

EE claimed a personal illness beginning on 8.22.16. EE missed time for illness. On 5.1.17 an email from the company outside attorney came claiming the illness is a possible work related illness or aggravation of an illness. EE is currently on restricted duty. Will amend when more information on case is known.

EE getting out of a Kawasaki Mule when EE stepped on a rock and twisted right knee. EE placed on restricted duty pending treatment.

EE operating a truck underground when the truck bed hit the back. The EE hit the steering wheel. Minor injuries at the time. Incident became reportable when EE was placed on restrictive duty on 8/18/16 with neck pain.

EE diagnosed with Carpal Tunnel Syndrome. Treatment is pending at this time.

EE alleged Carpal Tunnel Syndrome on 1.28.16. After rescheduling appointments multiple times, EE was diagnosed with bilateral carpal tunnel syndrome on 6.13.16. EE terminated employment on 5.15.16. Treatment is still pending at this time.

EE climbing into haul truck cab, strained right shoulder. EE was placed on restricted duty on 6/10/16.

EE performing maintenance on a drill. When exiting the drill, EE felt pain in knee stepping off the ladder. EE placed on restricted duty pending further testing.

EE felt pain in low back region while driving haul truck underground after hitting a pot hole. EE went to the physician on 4-15-16 and was placed on restricted duty.

EE diagnosed with Carpal Tunnel Syndrome and recommended surgery. EE remains at Full Duty until surgery, will then be placed on restricted duty following surgery. Surgery dates are pending at this time.

Bearing failure on generator that supplies power to amplidine to #2 Hoist, at 3:05 pm. MSHA notified at 03:17 pm. Repairs were completed and hoist returned to operation at 5:42 pm. MSHA was notified at 5:43 pm.

Electrical components failed in PLC cabinet that controlled the dynamic brake for #6 hoist motor. MSHA notified at 4:55 pm. Repairs made and hoist was back in operation at 8:00 am on 10/29/2015. MSHA notified at 8:01 am.

EE using an air hose to clean and got debris in his left eye. EE was given a prescription eye drop and released back to work with no restrictions.

EE was walking across the park underground when he tripped on a rock and felt pain in hip. EE placed on restricted duty pending further testing.

Using hook knife to extract cable in conveyor belt for splice, knife contacted leg requiring 4 stitches.

EE was moving a piece of steel when it hit him on the ankle. Diagnosed as sprained ankle, placed on restricted duty.

EE was driving a haul truck underground when his light belt hit the bed lever, the bed raised and hit the back suddenly stopping the truck. When the truck stopped suddenly, EE hit his mouth on the steering wheel causing a laceration. EE received stitches and was returned to work at full duty.

EE was changing a dart valve on the CU rougher, the cap on the dart actuator was not secured and flew off hitting EE in the mouth. EE broke a tooth and chipped another.

While EE was blowing dust and debris with air hose, EE got something in left eye. Dr removed small piece of metal imbedded in left eye.

EE got foreign body imbedded in left eye. Piece was removed and prescription eye drop was given to EE.

Employee shut his hand in a haul truck door, laceration to left thumb requiring stitches.

Employee was loading tools into pickup truck and jumped from tailgate to ground and injured right knee

EE was sweeping off shelves and counters and got debris in right eye. EE went to dr and received prescription eye drop.

EE smashed pinky finger on left hand between train car and scaling bar trying to remove a boulder. Company doctor removed the nail.

Occupational illness - alleged carpal tunnel in right arm and hand

EE was walking down front steps of the office that was covered with snow when he twisted his knee. EE had surgery on April 16, 2013 making this a reportable injury.

EE was servicing locomotive when the spring loaded handle pinched his finger resulting in a fracture bone and stitches.

EE was repairing a return roller on the #36 Feeder Belt when he stood up he felt pain in his knee. He had surgery on June 12, 2012.

EE was loosening a large nut on a haul truck when the impact socket broke and fractured his left thumb.

EE did not realize that wheel he was removing was a split rim that came apart and fractured his left hand.

Running 3""casing. While making a connection, employee let down winch line before connection was made smashing the employee's middle right finger with the 3""casing.

EE was cutting wires on a compressor when he cut his left hand with a razor knife resulting in stitches.

Tripping out drill pipe and 1st helper threw rod to 2nd helper on the rod truck. Drill pipe hit a stake on the side of truck that was left in and bounced over and landed on 2nd helpers foot causing him to fracture right big toe.

EE was removing a piece of metal when the metal cut his hands through the gloves resulting in stitches.

EE was hammering bolt, sliver of metal broke off and struck EE's left forearm. Employee received stitches making this reportable.

EE had tingling in both his left and right hands and forearms on 7/30/09. EE was diagnosed with carpal tunnel syndrome and had surgery on 7/1/2010.

EE alledged repetitive motion over a period of time to right arm, elbow and shoulder and has caused pain/carpal tunnel. EE has been being treated and on 12/23/09 he was given an injection to right shoulder and was prescribed physical therapy, making this reportable.

Employee was pulling pump coupler off front of engine when foot slipped causing him to go down on hood. When he went down on hood a bolt was sticking up and it struck left testicle. He has been treating continuously with co. dr and specialist and he was given an injection and taken off work effective 3/2/10. He will have surgery on 3/10/10.

Employee was dumping 50 lb bags of anfo in anfo pot on truck when he felt sharp pain in back of neck. He was treated by company physician and later referred to a specialist. He received injections and will do physical therapy.

Employee drove truck through chug holes and felt pain from neck between shoulder blades down back to belt line. Employee was taken to emergency room and was given an injection in neck.

Employee was walking along side of crusher and got dirt in right eye. Employee was taken to company doctor on 6-30-08 and received antibiotic eye drops.

Employee was working around ore bin when particle blew in right eye. Employee was taken to company doctor on 6/5/08 & a foreign object was removed. Physician also prescribed antibiotic eye drops.

Employee was using 8 lb. hammer to hammer on undersized chute to get it clear when his right shoulder popped and he felt sharp pain. Employee was seen by orthopedic physician and received a cortisone injection on 09-23-08.

EE had been hosing 1040 beltline, walked back to top to drain hose and water lines, slipped on ice on the walkway backdown. EE had a hold of the hand rail which caused him to swing into ramp wall, hit ramp wall with hip on angle iron.

Employee was cutting a rubber hose with utility knife. Knife slipped and punctured/cut left arm between wrist and forearm. Employee was taken to company doctor where he was treated. Employee received 3 sutures.

Employee was using wire wheel to clean up some parts for a tractor when a piece of wire came off and went into his left forearm. He later felt pain and reported accident on 12-06-07 and was then taken to company doctor. A specialist was not able to remove the piece of steel. He decided to leave it in his arm as it could do more damage to remove it. Employee received 6 sutures.

While stepping off grader, employee's right knee popped. After continuous medical treatment, Employee had surgery on 1/24/08 making this reportable.

Safety dogs on man cage would not move properly. Called Rolla Office at 3:00 PM. Reported hoist down. Pulled everyone out from underground and MSHA said to call back the next morning. Called MSHA on 9/12/07 AM that hoist was back in service at 1:00 AM.

Employees elbow became swollen and re-aggravated from a previous injury. Employee was taken to company doctor on 8-29-07 and given a prescription for anti-inflamatory.

Employee was hand scaling pulling on a slab from back in m3 and strained too hard. He was taken to company dr and referred to specialist. He began loosing time as of 7/20/07 and is scheduled for hernia surgery on 7/25/07.

EE was standing on old conveyor belt when the belt came apart & EE fell to floor. He was taken to Ellington ER on 7-8-07 & released to return to work. He was taken to co. dr. for follow-up on 7-9-07 where he was prescribed pain med. & muscle relaxer. He continued to complain of pain & was taken back to co. dr. on 7-10-07 where add'l x-rays were taken. EE was released to full duty.

Employee was hand scaling back when a small piece of rock fell on his face and right eye. He was taken to company doctor and prescribed an antibiotic.

While EE was putting brake booster in loader arm was extended too far back & right shoulder popped. EE taken to company dr, and later referred to specialist. EE rec'd cortizone shot on 5/31/07 making this reportable. After continuous treatment he had surgery on Jan 7, 2008 making this a lost time accident.

Employee was getting off 08-91 tractor & twisted right knee. EE was taken to company doctor & later referred to orthopedic specialist. EE had torn meniscus & had surgery on 3/23/07 to repair.

Employee went to remove lock out and turn breaker on. When he turned on the rod mill breaker it blew up blowing breaker doors open. He burnt his right arm and bruised right forefinger. He was taken to co dr and prescribed an antibiotic ointment for the burn.

EMPLOYEE WAS HOLDING THE 500 MM CABLE WHILE HIS PARTNER WAS CUTTING THE SHEETING WITH A UTILITY KNIFE. KNIFE CUT THROUGH THE SHEETING CUTTING EMPLOYEE'S RIGHT HAND BETWEEN KNUCKLE OF INDEX & MIDDLE FINGER. EE WAS TAKEN TO E.R. WHERE HE WAS GIVEN 7 SUTURES.

EE PULLED INTO HEADING TO INSPECT GROUND CONDITIONS AND MUCKPILE. HEARD A CRACK AND A LAYER OF LOOSE FELL FROM BACK SMASHING THE ROLL BAR ON THE KUBOTA AND THE EMPLOYEE. EMPLOYEE RECEIVED A LACERATION TO FACE AND A BROKEN NOSE

EE PULLED INTO HEADING TO INSPECT GROUND CONDITIONS AND MUCKPILE. HEARD A CRACK AND A LAYER OF LOOSE FELL FROM BACK SMASHING THE ROLL BAR ON THE KUBOTA & INJURING THE EE. EE REC'D SCRATCHES & CHIPPED TOOTH.

EE WAS ADJUSTING FORK WIDTH BE LIFTING AND SLIDING FORK TINES ACROSS ADJUSTING BAR. FORK TINE SLIPPED FROM LEFT HAND AND PINCHED RIGHT INDEX FINGER AGAINST THE FORK FRAME RESULTING IN A 4"" LACERATION FROM KNUCKLE (2ND) DOWN. REQUIRED 12 SUTURES.

EMPLOYEE WAS REMOVING RUST FROM A PUMP STUFFING BOX WITH A CHIPPING HAMMER. WHEN HE TOOK OFF HIS HARD HAT & GLASSES A PIECE OF RUST GOT INTO HIS RT. EYE. EE WAS TAKEN TO COMPANY DR. ON 11/01/04 WHERE A PIECE OF RUST WAS REMOVED. HE WAS GIVEN AN ANTIBIOTIC TO PREVENT INFECTION.

EE WAS GETTING A DRILL STEEL AND STEPPED ON A PIECE OF OLD BANDING THAT EE DID NOT SEE. EE CUT THE INSIDE OF LEFT FOOT BETWEEN ANKLE AND THE ARCH. EE RECIEVED 3 SUTURES.

EE WAS PRYING ON A PIECE OF EQUIPMENT WITH SCALING BAR WHEN HE FELT STRAIN TO LEFT ELBOW. EE WILL START 1ST SESSION OF PHYSICAL THERAPY 6/3/04 MAKING THIS REPORTABLE.

EMPLOYEE WAS LIFTING ON FLOOR GRATING WHEN HE FELT PAIN IN LOWER BACK. EE CONTINUED TO HAVE PAIN & WAS SEEN BY DR. ON 6/2/04 AT WHICH TIME HE RECOMMENDED PHYSICAL THERAPY. P/THEAPY BEGAN ON 6/23/04 MAKING THIS A REPORTABLE INJURY.

EE WAS FIXING FEED CHAIN, HAND SLIPPED WHILE PULLING CHAIN, HITTING HAND ON FEED RAIL. LACERATION TO RIGHT WRIST REQUIRING 8 STAPLES.

EE was coming down the steps to check zinc feed pump when he stepped onto slick floor and lost footing. ee strained left knee and ankle. Employee will have 2 surgeries-1st one on 6-7-04. for the knee and 3 weeks later 2nd surgery on 6-28-04 operation on ankle. est lost time is 3.5 months

WHILE REPAIRING LIGHTS ON CAT HAUL TRUCK, EE WAS CUTTING INTO AN OLD WIRING SPLICE. AS HE CUT THRU INSULATION WITH KNIFE, KNIFE SLIPPED AND CUT HAND. LACERATION TO LEFT HAND REQUIRING 4 SUTURE S.

THE EE REPORTED WHILE CARRYING A JUMBO TRAILING CABLE PLUG FROM PIGTAIL EXTENION BACK TO THE PLUG HOLDER ON THE JUMBO DRILL HE TRIPPED ON A ROCKAND FELL TO THE GROUND HE PUT HIS RIGHT HAND DOW N TO BREAK FALL A LACERATION OVER THE FIFTH FINGER KNUCKLE ON THE RIGHT HAND REQUIRING SUTURES RESULTED.

EE REPORTED WHILE CHANGING A TIRE ON A CATERPILLAR 938 LOADER IN THE UNDERGROUND SHOP, HE WAS PULLING DOWN A BREAKER BAR TO LOOSEN THE WHEEL STUDS. A CO-WORKER ADDED WEIGHT TO THE BAR CAUSING THE EE TO LOSE BALANCE AND FALL ON HIS KNEES RESULTING IN SWELLING OF THE RIGHT KNEE. LOST WORKDAYS BEGAN ON 09-02-03.

EMPLOYEE REPORTED WHILE SERVICING THE EQUIPMENT AND TRYING TO KEEP HIS FEET DRY, HE SLIPPED AND FELL CUTTING HIS LEFT FOREARM ON A SHARP ROCK ON THE RIB. EMPLOYEE WAS TRANSPORTED TO SALEM EMER GENCY ROOM FOR A LACERATION TO THE LEFT FOREARM REQUIRING SIX SUTURES.

EMPLOYEE ALLEDGED CARPAL TUNNEL SYNDROME TO BOTH LEFT AND RIGHT WRIST. EMPLOYEE HAD SURGERY TO THE LEFT WRIST ON 9/18/02, AND ON 9/25/02, EMPLOYEE HAD SURGERY TO THE RIGHT WRIST.

THE EMPLOYEE REPORTED WHILE CHANGING A STRIKER BAR ON A ROCK BOLTER, HE USED A HARD METAL HAMMER TO LOOSEN IT. A METAL CHIP BROKE OFF OF THE STRIKER BAR IMBEDDING IN HIS RIGHT FOREARM. A DR. REMOVED THE OBJECT AND CLOSED THE WOUND W/2 STITCHES. THE EMPLOYEE RETURNED TO FULL DUTY WORK ON THE SAME DAY.

EMPLOYEE WAS LOWERING A LINER WITH A ROPE AND FELT PAIN IN LOWER BACK. LINER WEIGHED 150 LBS. AND WAS BEING LOWERED APPROXIMATELY 2 FEET INTO TRIPPER CHUTE ABOVE FINE ORE BINS. PHYSICAL THERAP Y BEGAN ON JULY 2, 2001.

WE HAD REPEATED POWER FAILURE DUE TO INCOMING VOLTAGE LOW. THIS CAUSED THE DYNA-BRAKE TO LOOSE ITS MEMORY. RE-PROGRAMED DYNA-BRAKE. MSHA WAS NOTIFIED AT 12:15 PM THAT THE HOIST WAS DOWN AND WA S CALLED AT 1:00 PM WHEN BACK IN SERVICE.

EMPLOYEES WERE REMOVING A BAD SECTION OF PIPE USING HYSTER FORKLIFT TO SUPPORT ONE END OF THE PIPE THAT HAD TO BE CUT OFF. INJURED WAS SUPPORTING THE OTHER END OF THE PIPE WHILE EE REMOVED T HE CLAMP, WHEN CLAMP WAS MOVED, THE PIPE ROLLED OFF THE FORKLIFT AND SMASHED FINGER BETWEEN PIPEAND ROPES ON FORKLIFT.

OTHER SOUTH MINE TRAIN OPERATOR WAS RETURNING SOUTH TO GET ANOTHER LOAD. TRAIN WAS TO SLOW DOWN TO A CRAWL AT #2 SWITCH CURVE HEADED FOR #4 RAISE TO LOAD ORE. TRAIN WENT PAST 32E/713 FUEL BAY THE LOADER OPERATOR PULLED ACROSS THE TRACK HEADED TO FUEL BAY AT QUITTING TIME AND WAS STRUCK BY EMPTY RAILROAD CAR.

EMPLOYEE STARTED SCALING BACK AND TOP OF RIBS WHILE WORKING THEIR WAY DOWN THE PILLAR, EE WAS STRUCK ON TOP OF THE HEAD BY A PIECE OF LOOSE 8"" BY 10"" WEIGHING AROUND 20 LBS. AND IT FELL ABOUT 10' TO 15' BEFORE IT STRUCK HIM. BEGAN LOST TIME6-01-01.

EMPLOYEE STEPPED ON A ROCK TWISTING HER RIGHT ANKLE RESULTING IN A HAIR LINE FRACTURE TO SMALL BONE IN RIGHT ANKLE. ANKLE HAD PREVIOUSLY BEEN FRACTURED RESULTING IN CHRONIC ANKLE SPRAINS. DR R ECOMMENDED NO CAST OR SPLINT AND ISSUED PAIN MEDICATION ONLY. EMPLOYEE WAS RELEASED FOR REGULAR DUTY.

TWO EMPLOYEES WERE USING THE UTILITY TRUCK TO HANG VENT BAG IN 696 N/E-850. ONE EMPLOYEE RAISED THE BASKET UP WHILE HOLDING VENT BAG IN OTHER HAND. HE HEARD INJURED CALL FOR HELP, TURNED AND SAW HIM PINNED BETWEEN THE WORK BASKET RAILING &THE BACK.

WHILE HAND MINING HE PUT PRESSURE ON SCALING BAR AND FELT PAIN IN HIS LOWER BACK. X-RAYS WERE NEGATIVE, BU EE BEGAN PHYSICAL THERAPY ON 10/11/00.

POWER KICKED OFF #43 HOIST. MSHA WAS NOTIFIED. HOIST REPAIRED AND BACK IN SERVICE AT 12:45 PM

POWER KICKED OFF #43 HOIST. MSHA WAS NOTIFIED. HOSIT REPAIRED AND BACK IN SERVICE AT 4:00 PM.

MAN HOIST FIALED TO OPERATE DUE TO FAILURE OF MICO SWITCH WHICH WAS REPLACED. REPORTED TO MSHA BY PHONE. HOIST WAS DOWN FROM 10AM TO 12NOON

EE CLAIMS WHILE SCALING OVERHEAD, HE PULLED MUSCLE IN LOWER BACK. FIRST DAY OF LOST TIME IS 1/24/00. (MODIFIED REPORT BASED ON DOCTOR'S DIAGNOSIS).

DURING INSPECTION ON #2 SHAFT, WE DISCOVERED THE BRACKET BROKE ON SOUTHEAST DOG ON SOUTHWEST CORNER. BRACKET APPEARS TO BE BROKE DUE TO YEARS OF SERVICE, METAL WEAR AND METAL FATIGUE. MSHA-ROL LA WAS NOTIFIED AT ABOUT 3:00 PM.
