196 on fileThe employee was cleaning up used vac hose (6"" corrugated piping) when they pushed some filled piping over the the toe plate and as it was going over the end whipped up and slung cement dust into their eyes. Prescription drops prescribed.
Debris and other unnecessary work material were present in the work area.
Employee had finished up using a grinder, too EE's face shield and welding coat off when they felt something in the eye. EE was seen by the local clinic, it was a small piece of metal that was removed with a cotton swab and they gave EE antibiotic eye drops.
Employee was operating an air hoist when their hand got caught in the chain of the hoist. It smashed is left thumb and index finger causing lacerations.
A contracted carrier loaded with cement was traveling on the plant road. The driver was speeding and ran off the road causing the truck and trailer to tip onto its side. The driver sustained a laceration to the head and was transported to a local hospital where stitches were required and a cracked vertebrae was discovered. Driver was also not wearing a seatbelt.
Employee was tasked to set the classifier. During the lift, EE's right thumb got pinched. EE was taken to the local ER and treated.
EE was using wire wheel to clean one of EE's welds. EE was wearing a long sleeved shirt and a welding jacket. One of the wires stuck into EE's clothing on EE's left forearm. EE accidentally leaned into the wire stuck in EE's clothing, pushing it into EE's left forearm.
While replacing a return roller, the employee loosened the bolts and the roller shifted and smashed the ring finger resulting in a laceration that needed medical attention.
While trying to install piping for heat exchanger, the pipe shifted and the individual had hand in the proximity of the pipe. The employee's left, ring finger suffered avulsion at the tip from the pipe and pinch point of a piece of support steel/gusset
EE was working with EE's crew to remove a motor from a piece of damaged equipment from the top of the barge/rail load out silos. When the motor came off the shaft, it swung away from EE, but then came back towards EE. EE placed EE's foot out, and the motor struck EE's right ankle pushing it into the beam EE was standing on.
Fire burned the tube belt and caused a galley to fall. Called in immediately to MSHA
Employee slipped in the ice and caught self before falling. Strained the right shoulder. Physician prescribed physical therapy on 2/12/25 making it reportable.
Employee was working outside during a windy, dry day and got dust in the eye. They didn't notice an issue until the next day when there was some irritation. They eye doctor prescribed antibiotic eyedrops for precaution.
Employee had surgery for a degenerative shoulder issue. EE will be on temporary modified work duty until released.
Using a hole saw, the spring popped the cut piece of metal out striking the finger and causing a laceration.
Pulling up on a bag house bag, when it broke loose the employee hit their face with their hand chipping a tooth.
Technicians were assigned to cut plate and weld it to repair the drum wheel on the magnetic separator. The tech had used brake cleaner to remove the grease and dirt from the face of the drum. When the arc struck fire burned the employee's hand.
Employees were tempting to install expansion joint and place the chains to rig it while employee was tidying it up it came lose and caught employee's hand.
Employee was bent down and kneeling for a long period of time. When they stood up, they felt a pop in their right knee.
Employee was working to clear a fly ash blockage when dust came out of the hatch and getting in the eye. The eye was flushed and antibiotic drops were prescribed.
Employee was walking across the rock floor in the dome and EE's knee twisted while walking. Prescription was issued 1/08/24 resulting in this becoming reportable.
The employee was hitting a bearing race with a hammer trying to get it to break free. A small piece of the metal race broke off and lodged in the upper right leg.
Employee was using a straight torch to cut a bearing. The torch went out from slag build up and the employee rubbed the torch tip EE's hand on which EE had a welding glove to knock slag off the tip. When EE did, the torch re-ignited and burnt a small hole in EE's glove and the inside of the glove melted to EE's hand
Technician was performing regular duties while drilling build up on the preheater tower and EE's knee gave way while holding the drill.
The coal feeder became overloaded.The Supervisor manually pulled the drive belt. The feeder struck the obstruction, reversing the direction of the drive belt and causing the Supervisor's right little finger to be pinched between the feeder's drive belt and pulley resulting in an avulsion injury.
Employee was handing the stationary vanes into the separator cage to another employee on the inside of the cage, then helping EE install the vanes. Employee didn't feel any pain at the end of the day, but noticed the next morning a sharp pain that runs from EE's neck to their hand. Injury became reportable on 5/04/22.
Employee was tightening conveyor belt. As they were climbing down off of the work area the employee's hammer loop on employee's pants caught one of the bolts, knocking them off balance, and causing them to land on their right leg funny resulting in a non-displaced fracture. Reportable on 2/1/22.
Employee was poking the feed shelf after completing climbing the ladder. While looking up, material that was on face shield fell down into EE's left eye.
Employee was walking around in the scraper compartment of the mill and slipped in the reject chute. As EE slid down the chute EE's left arm (shoulder) overextended quickly. Reportable on 12/1/2021.
Employee was using a dead blow hammer hitting a sucker hose to free material. EE felt sharp pain in the palm near right thumb. Incident became reportable on 11/12/2021.
Employee was tasked to test a High voltage cable. While preparing the cable to be tested, employee stepped down on a slope to get a piece of wire, losing footing and spraining left ankle. Incident became reportable 11/1/2021.
Employee and coworker were rotating a magnetic separator belt. The coworker lost grip on the belt causing the injured employee to lose balance and grip resulting in a backwards same level fall into a handrail and a fractured rib.
Employee was dislodging a rock from pneumatic gate. When the rock broke free the gate moved suddenly pinning employee's hand between the shovel EE was using and the wall of the building resulting in a finger laceration.
Technician was shooting industrial gun on the tower which required technician to have the gun at a steep angle. Thumb was required to be on the back of the gun as that is where the safety is located. When the technician fired, the recoil of the gun smashed right thumb between the gun handle and the refractory for the door resulting in a fractured thumb.
Employee removing all-thread when the assembly gave way causing EE's index finger knuckle to hit the bearing guide resulting in a dislocated right index finger.
Employee picked up a chunk of hard material and was carrying it to disposal when chunk slipped out of hand striking employee on the right foot above the steel toe. Diagnosis is fracture of right little toe.
After replacing segments on 491-PN1 head sprocket employee started feeling pain in left shoulder. Incident became reportable on 2/24/21 due to medical treatment.
Employee was using jackhammer to break material up. Employee noticed the next day their hand was hurting. Became reportable on 3/9/21 due to physical therapy.
Employee was climbing out of vac truck to turn the vacuum truck operation on when they stepped off the step of the truck they felt a popping in a knee and almost fell down. Diagnosed with a torn meniscus in left knee.
Technician was cutting duct tape from sucker truck piping with a razor knife when it punctured and cut through EE's left glove and hand resulting in a laceration.
While installing a lid on a 5 gallon bucket with a spud wrench, employee was hammering the lid down and the wrench slipped out of EE's hand and jammed into the hand in between EE's thumb and index finger resulting in a laceration of the right hand.
Technician reported that while shoveling spillage from around the CKD work bin technician felt a twinge in left abdomen. Technician has previous history of hernia surgery in same body location. Diagnosis is hernia.
Employee was removing a 3 inch water hose from the quarry's water truck after filling it. As employee was doing this employee lifted the hose to shoulder to then put it on the ground. This fitting is approx. 5 ft above the ground, & the hose was partially filled with water adding to its weight. The employee said employee felt a pop in shoulder followed by pain in shoulder.
Completing a work order to replace the bags on L91-BF3. The cages and bags were sized to the cookie sheet and while trying to remove them with channel locks with jerking motion, with two other people helping, employee strained right shoulder.
While walking up stairs and carrying 12 liters of oil, employees knee gave out and became unstable. Employee provided with physical therapy on 6/8/2020 making reportable.
On 9/9/19 employee was assigned to perform annual lube of a pan conveyor. Technician reached down to pick up a 5 gallon bucket of grease and felt 3 pops in back. The incident became reportable on 10/10/19 due to medical treatment.
Technician was changing bags in baghouse. After removing approximately 20 bags the technician reached down to pull out another bag when the technician felt pain in right shoulder. The bag became stuck on the way out causing the strain. The incident became reportable on 6/14/19 due to assigned physical therapy.
Employee was assisting another employee with a tower plug. Employee was unaware that kiln dust had accumulated on face shield. The dust fell into employee's right eye.
Employee was freeing material from a chute when a large amount of dust released making the area hard to see. As the employee tried to exit the area, EE fell on left shoulder resulting in a strain. The incident became reportable on 12/10/2018.
Employee was dismounting dozer when foot slipped causing them to lose grip on the handrail and falling to ground on left hip and elbow. Employee diagnosed with fracture of elbow on 8/28/2018.
EE was tasked with installing compressor replacement on A/C Unit. As EE positioned compressor into the unit, EE's left middle finger made contact between compressor and surface.
The employee injured knee as EE fell after being struck by the end of a belting roll. Employee was un-banding the roll preparing to un-wind the roll and didn't realize the roll end was on top. The free end unexpectedly slipped down when un-banded and struck our employee causing EE to fall and injure left knee.
Employees were reassembling guards on a belt conveyor. The injured was sliding a guard back into place when left hand was pinched between the guard and a handrail resulting in a contusion. The incident became reportable on 1/2/2018 due to physical therapy being assigned.
The site was lowering vanes from a coal mill via a pulley and rope system. A vane fell against the leg of the employee injuring EE's left knee.
Employee was a part of a crew that was assembling portion of a conveyor chain. To assemble the chain, pins must be pushed out hydraulically with a pin pusher. Employee felt pain in left shoulder after lifting pin pusher. Injury became reportable on 8/9/2017.
Employee was filling 55 gallon drums with cement. As the employee tried to cap off the discharge line, a surge of cement caused the cap to dislodge and cement to enter the employee's eyes.
Employee was tipping an electrical cabinet (approximately 300 pounds) from vertical to horizontal with another employee when EE felt pain in EE's right forearm. Injury became reportable on 4/22/16.

Employee was on a stepladder when the ladder shifted causing the employee to strain his left shoulder. On 7/9/15 he was diagnosed with a left shoulder rotator cuff tear.
Employee was at the tenth section of staves on the silo. An existing structure did not allow the employee full access to the scaffolding. While the employee was installing bolts on the chime, he overshot his swing causing him to lose his balance and fall forward. When retracting back, he caught his arm on the corner of the stave lacerating his left forearm.
Employee was using hand drill when drill bit grabbed causing the drill to twist in the employee's right hand. This resulted in a fracture to right hand.

Employee was moving a barge with a winch cable when the barge shifted causing the cable to strike the employee in both shins causing abrasions. Employee did not seek treatment until 01/25/2012 when it was discovered that the abrasions had become infected making the incident reportable.

The security officer left the guard shack to get a breath of fresh air. Within a few feet of the guard shack, the officer slipped and fell on a piece of ice. When the officer's shift ended, he was treated by medical professionals, found to have a fractured wrist but was released to go back to work. Therefore, no work hours were lost due to the injury.

Employee was moving flashing and when he went to set it down it rolled over in his hands. In an attempt to catch a new grip, the flashing landed on his knee cutting his leg just above the knee.

A 98"" x 24"" corrigated sheet slipped off a C-11R vise clamp falling approximately 30 feet striking injured in the right facial area. Injured received severe laceration from right ear to mouth.

Employee stepped over a pipe approximately 4 ft in the air, employee thought the pipe was supported by the blocks under the pipe. When his Left foot went over the pipe, and he put in down on the blocks, the blocks were not supported and blocks moved causing employee to fall, fracturing his Left ankle.
Individual was cutting grating in a very tight spot, when he lost control of the 4"" grinder and cut his right hand. Individual was supporting his weight with the right hand and grinding with left.
IW was moving/lifting a 3'x10' metal plate with a co-worker and sliding it back over an open concrete trench. The plate slid and fell 18"" down onto the IW's right foot. The IW sustained fractures to the upper part of his foot.
EE was ducking under some scaffold and raised up too soon, hitting his head on a pipe resulting in a laceration to right eyebrow.
While moving a well wheel used for hoisting siding panels, felt shoulder popand immediately experienced pain. Made hospital visit and returned to full duty. Upon follow-up visit, still complained about pain eventually resulting in x-rays, MRIs and now in 4/23/2009 a procedure to repair tendon damage.
It was snowing/sleeting outside and the temperature was around 28 degrees. The employee was descending extension ladder and slipped on rung, fell 10 feet. When employee fell the left leg was hung up on rung which caused a fracture to the left femur bone, and the employees head hit scaffold board which caused laceration to the left side that required 12 staples.
Worker was exiting her work area and slipped and fell on concrete floor. Worker reported pain in her right knee. Original x-rays on 2/19/09 showed no fracture. MRI on 3/31/09 revealed fracture to right tibia.
Employee was working with a partner cleaning up a work area. He and partner picked up a ""rail road tie"" to carry over to their buggy. Employee felt a strain in his back and was unable to get up without assistance. Approximate weight of tie was 100lbs.
Employee was walking to restroom at the start of the shift and slipped on ice. He broke his wrist trying to catch himself when he fell.
Slipped on a small patch of ice at the base of the scaffold, grabbing the scaffold to support fal; twisted his back. He reported onset of low back pain with radiation to lower extremities, spasm in lumbar spine; X-ray demostrated significate degenerative arthritis. Which was aggregrated by the incident.
EE was drilling a hole in refractory when the drill bit got stuck and the drill twisted out of EE's hand. This resulted in a fracture to the fourth metacarpal.
Employee was breaking loose bolts on steel structure with a wrench, when he felt a pain in his right shoulder and neck.
EE was removing a trash hopper from a freight elevator. The elderly elevator operator was sitting in a chair inside the elevator and refused to move, so in an effort to avoid hitting the operator, EE caught small finger between hopper and elevator, causing a fracture to the tip of the right small finger.
Employee was pulling the starter rope on a small generator. The engine compression was greater that the pulling power resulting in a backlash on the rope. The backlash pulled his fingers apart from the ring finger, splitting the web at the top of his left palm. He was wearing cotton gloves at the time of the injury.
EE was working hoisting a large valve from level 4 down to 3. They were using a 10 ton chain hoist to lower and were using their legs to push the valve out to clear the handrail. While pushing against the valve he felt his knee ""buckle"" from the pressure. Resulted in a dislocated knee cap. Knee cap was manipulated back into place in the emergency room, resulted in a knee strain.
Employee was assisting in refractory installation when he bent down and felt a pain in his back.
Employee was working on a conveyor catwalk. He got into a hurry while traveling the catwalk, tripped over a spool that was holding wire that was being installed. He fell to the catwalk, resulting in a laceration to his shin and strain to his shoulder. Laceration required 3 sutures.
Employee was in a scissor's lift pulling cable. He felt a pain in his stomach, lower mid section.
Individual slipped off a 12 in. dia pipe he was standing on while operating a chain hoist striking his elbow on the corner flange of the duct work resulting in a deep contusion and bursitis in the right elbow. Individual was treated by the site nurse and then sent to the Doctor where it was eventually drained and injected with steroids.
Employee was descending a permanent ladder from control room inside C/A building when he slipped off the ladder and fell backwards striking a pallet/box with his mid back. He fell a total of approximately 6 feet to the ground below. Employee does not recall a specific cause, ""I just slipped coming down the ladder"".
A pipe fitter working on the removal of some 1"" diameter piping was struck on the hand when a piece of pipe he was removing fell and struck his right hand.
Employee was exiting a man lift when he stepped on a rock and turned his ankle. Conservative treatment was persued for 2.5 months and had outpatient surgery to repair tendon on 3/6/09. No lost time, restricted duty.
Employee became disoriented while performing firewatch duties. He stepped off a 34 inch high platform knocked over a barricade and fell to the ground. Employee claims a reaction to medication caused him to become disoriented. NO INJURIES HAVE BEEN SUBSTANTIATED AT THIS TIME.
Boilermaker was using a 10 ton port a power and a ""L"" shaped lug to align 2 sections of ductwork. Weld on the lug broke under pressure, causing the lug to bounce up and strike the boilermaker in the face. Resulted in a laceration to his upper lip that required sutures to close.
Employee was drilling a hole for an ice melt spreader mounting bracket, the 1/2"" drill bound up when the bit broke through the metal plate causing the drill to spin. Employees little finger was caught between drill and frame causing a fracture of his little finger.
While extending his upper body out of the wall to clamp top of siding panel into place, the employee rolled over on his side quickly to move out of the way of panel being hoisted. The toe plate he was lying on contacted his ribcage in a manner that caused bruising & much soreness. X-rays were taken to check for broken ribs (none).
Employee was welding and grinding on hopper section. Employee finished grinding and went to put on welding hood. When employee put on welding hood, employee felt a foreign body in his left eye. Employee did not report injury immediatley, which may have caused the infection to develop in his eye. The injury was reported approximately 4 hours after the incident.
Employee struck his knee on stacked handrail waiting to be installed. From the contusion on his knee it got infected and fluid had to be taken off his knee.
EE was untangling an extension cord when another worker (employed by a different contractor) dropped an unsecured chain fall onto him. EE was struck on his hard hat and the chain fall also struck his shoulder. The injury became reportable on 10/1/08 when physical therapy was prescribed.
Employee was moving plywood off of equipment skid, when he stepped on a rock. This twisted his foot/ankle, casuing a chip fracture in his left foot. Employee reported on 9/22 and visited the Dr. on 9/24.
Employee was erecting scaffold inside of G-Ring approximately 5' off the ground. Placing boards onto brackets the board slipped and as he compensated for this, the end of the board pushed his hand onto the steel plate mashing and cutting his left thumb. He was wearing gloves and all appropriate PPE.
Employee was assisting in the dismantlement of a scaffold. While passing a wood plank to a co-worker below, he received a splinter under his upper arm.
While cutting plastic band off of door hardware, the utility knife slipped off of the plastic band and cut his his left thumb.
Employee was torquing bolts using a torque wrench. He used his shoulder to push the wrench upward causing a compression injury to his shoulder. Injury was not diagnosed as a tear to tnedon initially and after months of continuing issue he has elected to have surgery to repair.
EE was working with hand railing, when wasps came out of railing. EE was stung on both forearms. Later in evening, EE's arms started to swell and the next morning the EE was taken to the clinic & given a steroid shot for the swelling. EE did not miss any time & released by doctor after follow up visit for observational purpose on 8/20/08.
Employee was working inside an aerial boom lift with a partner. They were in the process of moving away from the structure, when the lift came in contact with the structure. Employee had his hand on the lift guardrail and pinched his hand between structure and guardrail.
At approx. 9:45 am an employee of Atlas/PMI who was working at the kiln piers as an operator was exiting the restroom on the south side of kiln pier 2 and was returning to his crane. As a result of the pathway the employee choose he tripped over a ramp to small connex box and fell.
Individual complained of having something in his eye. He went to the nurse. The nurse removed 2 pieces of debris from his eye and could not remove the third. Sent him to clinic and his eye had to have several layers scraped to remove piece.
Employee was moving steel braces for structural steel erection by hand and strained his back. Injury has not improved and increased treatment went into effect on 8/26/08 causing injury to become reportable.
It had previously rained and there was standing water at the base of scaffold. When employee started climbing down the ladder, the welders gloves being wet, he lost grip with one hand and then the other and fell to ground.
Brace lowered into place with crane, choker at upper end, come-a-long at lower end. Bolt put in upper brace to beam connection. Crane took over brace's weight, come-a-long was removed, attached in the horizontal direction to pull brace in place. As brace was pulled, bolt in upper end of the beam failed. Brace swung down, hit a hand rail, then employee on left thigh.
Employee was using a pry bar to adjust a steel frame, during steel erection sequence. Bar slipped out from frame and struck employee in the cheek, causing a small laceration. 4 sutures were required to close the wound.
Employee was welding on a kiln. The steel had to be heated up and the welder had to weld from one side of his body 10 hrs a day. A buch of loose skin devloped on his left elbow, he complained of the pain. Took him to Dr. and therepy was administered.
Employee was walking through an area that had been prepped for a concrete pour. He did not see the string line pulled to mark grade. Employee tripped on the string line and fell striking his knee on the ground resulting in a fractured knee cap.
Employee reached in fan housing to manually move fan blade for fan alignment. Fan had accumulated water and movement caused water to shift and the fan to move further than expected. This resulted in a pinch point injury to employees hand, when caught between fan blades and housing.
Individual was standing a beam up from the ground so it could be moved by an Industrial Powered Vehicle, when he felt a burning sensation in his left groin. Was not diagnosed as a hernia until 7/2/08.
While lowering handrail it got hung and fell/dropped. Employee tried to grab it and it landed on his right wrist and pinned against stationary handrail. Fell to ground.
Employee was walking on the side of a small hill, his foot slipped, and he fell sliding down trying to catch his fall and his right arm hit a rock. He received a small laceration to his right arm back side below elbow.
Employee was grinding a piece of 3""x6"" steel plate with a 5"" hand held grinder when the unsecured plate began to move. Employee reached down to grab the piece and struck his left thumb with the grinder, the resulting laceration required 5 sutures to close.
EE was pulling up buckets of bolts to work site during his shift. EE did not have any real pain in his shoulder and did not report the pain/injury to his foremen until shift start the next day. EE had surgery (13 years ago) to right shoulder. EE was placed on lifting restrictions of his right arm/shoulder by the doctor.
Employee was rolling a reel of 500 mcm wire off the forks of the Skytrack when the right side of the reel took off, causing the left side of the reel to kick back toward the back brace of the skytrack fork bed, pinching employees left middel finger between the fork bed and the reel
Employee states he felt a pain while lifting panels. Reported after 3 day holiday weekend.
Employee was working on a 12"" I beam, the job was finished he attempted to get off beam. When he did, his lanyard got hung on a handrail and pulled him off steel. He dropped approx 4 ft before retract engaged and he swung into the structure (cement silo). The conditions were wet it had been raining off and on all day, now it was coming down pretty good.
Employee was tightening bolts pulling on wrench he strained shoulder.
Boilermaker was sorting material from a shipping container, taking inventory. He was placing 7""x7""x1"" steel blocks onto top side of a beam and pushing them to the side to get a count. Employee felt a strain in his left shoulder. Reported on 4/28, became reprotable injury on 5/9 after MRI revealed a tendon tear.
Employee was pulling welding lead to roll up when it hung up on a piece of equipment and he heard a pop in his right shoulder. He tore muscle in right shoulder and has been restricted until swelling goes down.
Injured was in the process of removing floor grating to access steel beams below. He bent over and tried to pick up grating panels instead of using crane or grating hooks and strained his lower back.
The individual was walking to get a tool when he ran into a horizontal beam. The individuals safety glasses made contact with the steel causing them to slide up and the top of the lens caused a laceration above the right eye that required 5 stitches.
Employee was working in a kneeling position laying out for installation of equipment rails in corrective additives building. Employee noticed pain and swelling in his right knee. He tried to work through the problem which got better for a while and eventually got to be more than he could deal with and finally went to the Dr. on 4/22/08.
While grinding, Employee went to raise the grinder into a different position when the electric cord became entangled. His right hand slipped off the handle at the same he gripped hard on grinder and grabbed to steady himself with his right hand. The grinder wheel contacted his left wrist.
EE was torqing bolts with a torque wrench when the wrench slipped at the same time he was pulling trigger to tighten bolt. His right hand slid down the extension bar and his right thumb was pinched between the ""backer arm"" and the nut.

Individual has developed numbness and pain in both hands and it was discovered through a nerve conduction study that surgery would be required on both wrists and elbows for Carpal Tunnel Syndrome. The individual reported the numbness and pain following 1 day of using a pneumatic chipping hammer to do some concrete work.
Employee was inside a storage vault hanging chokers on a hook. He placed his left hand on the door jam while inside, door closed pinching his left middle finger. This resulted in a laceration requiring 15 sutures to close.
Placing a large shackle onto a lifting beam, employee placed his thumb in between shackle and beam while moving it into place, pinching the end of his thumb resulting an a fracture of his thumb.
Employee was on fire duty. While she was walking with another employee who was using a cutting torch, that employee slipped and torch went across the injured employee's right lower arm.
Employee was walking in work area, tripped over a chuck of dried concrete and fell down. Employee struck his face on concrete lacerating the area above his upper lip and below his nose. Laceration required stitches to close the wound.
An individual was working on the erection of an elevating scaffold. While lifting a piece of the tower over his head he felt a ""pop"" and experienced soreness. The individual saw the doctor where x-rays were negative but an MRI showed a partial tendon tear in his shoulder. The injury was initially treated through physical therapy but eventually had to be surgically repaired.
Inj. was welding and grinding on floor plates of pre-heater building. Does not know of a specific incident, noticed iritation in rt eye. Tried to wash out on his own, did not report to Alberici until 9-15, went to the Dr. Dr. removed piece debris without complications, follow ups on 9-17 & 9-24, became reportable on 9-24 due to presc. meds. No restrictions or lost time.
Iron worker foreman was cutting open a cardboard box containing bolts for steel erection. He was using a pocket knife with his left hand while holding the box with his right. Knife slipped and cut his middle finger on the right hand. Cut required sutures.
Labor was getting ready to move a cement slick line. Just as they were about to move the line, several saw horses that were supporting the slick line collapsed. When this happened, the weight of the slick line shifted toward the labor, causing him to get off balance and roll his ankle inward (inversion). The diagnosis was a lisfranc injury to the right foot.
Employee noticed a lump in his groin on the right side.
The employee was the pushing concrete placer when the 45? pipe ruptured. The employee was splashed or sprayed with concrete in the face and neck.
Injured employee was walking down a gravel embankment that was approximately 15"" high. The employee's right foot slipped away from her body and her left knee went down on to the gravel which caused a laceration.
Employee had been working for an hour and a half when he walked up to his foreman and indicated that he was dizzy. Employee passed out, falling on his front. Employee awoke a moment later, received facial abrasions/lacerations, diagnosis was heat exhaustion.

The employee was installing a # 4 horizontal section of rebar in a pilaster. The bar sprung back and struck the employee across the left shin just below the knee.
Employee was guiding post tensioning cable from the ground to prevent the cable from entangled. The post tensioning cable then shot out of the spool located approximately 15 feet from him and became entangled around itself. The employee?s hands became caught between the cables, causing him to fall to the ground.
Employee was operating the hose of a concrete pump, pouring a concrete wall when he felt a pain in his right knee. Employee has a strain with a possiable patellar tear.
The employee was installing a section of # 11 rebar when it got hung up on a trumpet for a post tensioning duct. He pulled the # 11 rebar down and as he did the rest of the crew pushed the bar forward. When the bar came forward his left hand middle and index fingers were pinched between a verticle and horizontal section of rebar.
Installing #9 Rebar when foot slipped down into the formwork and rebar at the end.
While installing #9 Rebar and feeding into structure, the rebar hit a ""C"" channel as he was pushing downward. The bar hit his wrist and his wrist twisted back and toward the right.
The Injured Worker was operating a rock crusher & had some dust fly into his eye. The IW flushed his eye out then used a kleenex to wipe his eye. He reported it to his supervisor around 11:40. Around 3:00 his eye began experiencing a burning sensation. The company then drove him into the eye doctor's office. The IW was wearing safety glasses at the time of the accident.
Speed of truck, inexperience of driver, wet haul road, and possible inattentiveness of driver - causing him to lose control of his 100 ton truck and slide into oncoming 65 ton truck (t-boning it). Both drivers were wearing seatbelts.
