Over-exertion NEC

Climbing up to put oil in the water scraper, EE's left foot got caught on the lip at the top. EE stepped up with EE's left foot and was bringing up EE's right foot and EE's left foot slipped. EE had a hold of the hand rail and tried to catch self, EE's right foot got caught on something causing the injury.

Caught in, under or between running or meshing objects

Employee was loading a truck with 200 lbs rip rap and one boulder fell off and they didn't notice it. They pulled forward running over the top of the boulder, that caused them to jar their back. Went back to work but as the day progressed so did the pain in their lower back and they notified their supervisor.

Over-exertion NEC

Was shutting off water pump after filling the water tuck. Was walking back to the truck, stepped on a rock, which caused them to stumble, they felt a pop and had some pain. Told a supervisor about it, thought they had sprain an ankle. As the day went on the pain became worse and they sought medical treatment.

Over-exertion in wielding or throwing objects

Was shoveling out mud out of the secondary crusher when the shovel hit the inside of the lip on the crusher causing the shovel to stop abruptly and bending their right wrist back causing immediate pain.

Caught in, under or between running or meshing objects

Was putting a drive belt on the crusher and was checking the underside to see if the belt needed to be rotated and the belt was put into reverse and EE's hand was on the belt and was pulled in between the belt and shiv, injuring EE's left hand.

Fall from machine

Was driving a haul truck when the truck in front of EE stalled. EE got out to see if EE could help. EE climbed up on the catwalk when the brakes disengaged and started rolling backwards. EE jumped off the haul truck and in doing so strained calf muscle and upon hitting the ground strained left wrist.

Fall from machine

Was stepping down off of a piece of equipment, had one foot on the step and one foot on the track of the loader. The foot on the track slipped and their Left knee popped.

Struck by flying object

Couple of boulders got stuck in the feeder. Employee walked up to catwalk to take a look and a rock shot out of the feeder hitting them in the chin on the right side causing a laceration.

Over-exertion NEC

Employee was climbing down off the loader, tried to turn around and EE's knee gave out and EE fell to the ground. Left knee is the injured area.

Over-exertion in lifting objects

Miner was shoveling off the cat walk and was twisting to throw it off the machine and felt it in EE's back.

Caught in, under or between a moving and a stationary object

Was operating a front end loader to load the shoot. A boulder became dislodged & obstructed EE's path to the shoot. EE got out of the loader, was pushing a boulder out of the way so EE could load the shoot. Pushed on it & it wasn't enough force. EE lowered stance & when EE pushed the boulder again EE's right index finger got pinched between the boulder & another rock.

Over-exertion NEC

Had a bar stuffed up in the bin trying to dislodge a boulder. The bar slipped and ee jammed left shoulder.

Struck by falling object

Was pulling a 20 ton floor jack out the back of truck when ee dropped it and it landed on left foot just behind the steel toe causing a contusion.

While driving an articulating truck at the quarry, something fell into EE's left eye.

Contact with heat

The miner was working outside in the heat. He entered the scale house complaining of being hot. He drank some water and complained of nausea and said he had stopped sweating. He was taken to K-stat to be checked out.

He was putting a V belt on the shive, when it rocked on his thumb got pinched.

Struck by flying object

Employee was changing the tooth on an excavator bucket. He was beating the pin out of tooth with a hammer and chisel when a piece of metal splintered off and struck him on the inside elbow of his left arm causing a laceration and embedding a piece of metal in the soft tissue.

Employee was power washing machinery and something got into right eye.

Employee was going down the ladder on a piece of equipment when his wedding ring caught on the ladder and hyperextended his left 4th finger and left wrist.

Caught in, under or between a moving and a stationary object

Employee used drive belt as a hand-hold. As employee pulled on the belt it moved; causing his hand to be caught between the belt and pulley. Equipment was NOT energized.

Employee was climbing down a ladder and slipped on a rung.

Fall from machine

Employee was fueling a bulldozer when he fell (approx. 3 feet) to the ground.

Fall to the walkway or working surface

Employee was walking down an icy access road when he slipped and fell flat on his back. He struck the right side of his low back and his head. He got back on his feet unassisted and continued to work the rest of the day with worsening pain to his back.

Caught in, under or between a moving and a stationary object

Employee was clearing a blockage in the drill bit of a hydraulic track drill. While unthreading the drill head, the drill stem came loose, causing the entire section to fall and crush his right ring finger.

Over-exertion NEC

Employee walked across parking lot when his knee went out. We did not initially submit this injury because there was much uncertainty about whether this injury was work related or personal (employee originally reported as non-work comp) and whether it happened at work or not.

Struck by flying object

Socket came off an impact wrench striking employee in the mouth.

Struck against a moving object

Employee was operating a haul truck that was getting loaded with large pieces of shale. She was wearing her seatbelt, but due to the load, was getting tossed around in the truck. After that she started to develop severe back pain.

Over-exertion in lifting objects

Employee was unloading powder and felt a small pain in his lower back. He had his back support on and was lifting properly.

NEC

Employee had come into the office several days prior and told us that he had been having chest pains and was going to see a doctor about it. When he experienced the pains at work he was immediatley taken to the ER. Employee returned to work the next day.

Contact with heat

Employee started throwing up, had numbing in the face, stopped sweating, had dizziness, and was disoriented due to extreme heat. Employee did not lose consciousness and was coherent at all times.

While crawling into a screening plant, a piece of rust fell off the screen into his eye.

While attempting to release tensionon a chain boomer. The boomer released hitting EE in the lower leg breaking his leg.

Fall onto or against objects

Ee was climbing a ladder to add antifreeze to an engine and the ladder started to move and ee fell approx. 5' landing on a 55 gal drum.

Fall from ladders

EMPLOYEE WAS PRYING ON A BOULDER WHILE STANDING ON A LADDER. THE PRY BAR CAME LOOSE AND HE LOST HIS FOOTING AND FELL APPROXIMATELY 4 FEET LANDING ON A ROCK.

Struck against a moving object

EMPLOYEE DRIVING LOADER ACROSS YARD AND HIT A ROUGH SPOT CAUSING THE LOADER TO ROCK SIDEWAYS STRAINING HIS BACK.

Over-exertion NEC

WHILE BOOMING A CONVEYOR TO BE OFF-LOADED, EE STEPPED FROM CONVEYOR TO TRAILER DECK AND ROLLED HIS FOOT WHICH RESULTED IN A SPRAINED ANKLE.

Struck by falling object

WHILE SERVICING TRUCK HE WAS LOOKING UP AT THE BOTTOM OF TRUCK AND DEBRIS FELL FROM BOTTOM OF TRUCK AND A FOREIGN OBJECT STUCK IN LEFT EYE.

Over-exertion NEC

EMPLOYEE, EMPLOYED AS A MECHANIC WITH BAYER CONSTRUCTION WAS CLEANING A RADIATOR ON AN EXCAVATOR. HE STEPPED FROM THE EXCAVATOR TO THE GROUND AND ROLLED HIS ANKLE WHICH RESULTED IN A SPRAIN. .
