Employee was walking down stairs. Felt a pop in their left knee.

IE stepped on a sheet of metal and it flipped off of the step and landed on their foot causing a fracture.

IE was carrying a top belt roller when EE's foot got stuck in the mud. IE then fell and landed on the roller hurting EE's back.

IE was pulling line curtain and twisted pulling a muscle in EE's back.

IE was using a pry bar to unplug a chute when a piece of rock rolled down the pile and hit the pry bar causing the pry bar to come down and smash a finger against the chute. IE received 6 stitches on ring finger.

IE was cutting a piece of wood with EE's knife when the blade slipped and cut self. IE received 6 sutures.

IE stepped from one side of the bolter to the other and the left knee popped while checking the torque on the roof bolts.

IE was strapping down materials on the tool sled when a piece of pipe rolled off the top of the sled and smashed the right thumb causing a fracture.

IE was assisting another miner with removing the drill rig operator's platform so they could change the drill bit. The IE got a finger pinched between two pieces of chain, resulting in a laceration that required 4 stiches to close up

An unplanned roof fall occurred in entry #3 in the intersection of 79 in HG 8.

IE was dropping tubes when the tube bounced off of the ground jamming EE's finger causing a fracture.

IE was trying to get the mesh off the tip of the shield when the shield tip came down and hit IE in the back causing a fracture.

Went to change battery on a scoop when the battery slid off of the cribs and hit IE in the knee

IE was looking in the dust box when a rib rolled out and hit them into the bolter.

IE was using a pry bar when it slipped and smashed THEIR left index finger between the prybar and the miner.

IE went to connect the bottom drill steel with the top steel when the top drill steel fell from the top and smashed their little finger between the steels.

IE tripped over a block/debris, fell, and hit EE's head jamming EE's neck.

IE was trying to free the pin on the scoop battery when EE hit EE's finger causing a laceration. IE received 5 sutures.

IE was taking the dust feed hose off of the pod duster and when they unhooked it, it whipped around and struck them.

IE was trying to sit back down in the dog box of a mantrip after sticking their head out the back. When IE brought their head back in, they caught their right ear on something and caused a laceration to the right ear.

On the pre-shift examination for the oncoming 3rd shift, the examiner discovered evidence of a heating event in the vicinity of xc 26. The operator was able to contain the condition by loading it on the belt and sending it out of the mine.

IE slipped on a shield toe carrying a PMCR and fell hurting their back.

Allegedly a rock fell out of the face in entry #4 and hit IE in the shoulder knocking IE to the ground. IE landed on a rock and injured their knee.

The Headgate drum on the shear threw a rock out that hit IE on the back of left ankle.

A piece of track pinched IE's finger against the pan line.

IE was standing next to a piece of plastic pipe so it would not hit a truck that was by the slope drive. The pipe got caught on something in the slope and when it broke free the pipe hit swept IE off of EE's feet. IE received 6 sutures on EE's bottom lip.

IE was connecting the 2 drill steels and the steels got in a bind. IE hit the rotation level to get the two steels together and the bottom steel slipped off the top steel and hit IE in the nose.

IE was lifting a 4"" slick line up to hand it when EE heard EE's shoulder pop. IE never missed a day of work till 10/28/2024

Torching out bolt off of a screen. The melted plastic popped. Sending a piece behind EE's glasses and striking EE in the eye. Had to get eye drops in eye.

IE was cutting down wire mesh. The mesh fell and cut IE on the forearm. IE received 5 sutures.

They were reeling up the cable slack on the bolter when the cable got tight and hit IE in the legs causing EE to fall to the ground hurting their back.

IE was pulling dust hose and tripped over curtain/mesh and fell causing a fracture in EE's ankle.

An unplanned roof fall occurred in entry #1 approximately 105 feet inby the intersection of crosscut 128 in the south mains.

IE was hanging cable on a rib when IE felt a pop in right leg near the groin.

IE was dropping belt structure when the belt rail fell and hit the belt. The rail landed on the IE left foot causing a contusion.

IE was stepping down from the bolter and sprained foot.

Employee slipped getting out of bobcat. Caught self with left foot causing strain to EE's middle back and left knee

IE was lifting a ventilation tube and tore bicep.

IE had hand full of tools and when IE went to set them down, the grinder turned on and cut IE's hand. IE received sutures.

cutting the sleeve back on the trunk line when the knife slipped and cut EE's wrist. IE received 6 sutures.

A piece of metal came off of the pick hammer when IE was changing miner bits. IE received 2 sutures

Hurt lower back carrying water pipe through a man door.

IE was lifting water pipe when the shut off valve slide off of the pipe. IE tried to catch the shut off valve and the valve hit IE arm against the belt structure causing a laceration. IE received 4 stitches.

IE was loading the arm of the bolter with roof bolts when they felt a pain in their back.

IE was moving belt structure when IE felt a pain in IE's back.

IE was working on the solid block stopping at XC 16 between entries 1 and 2 in HG 7. The wall was about 10 feet high when it started to fall over. IE was struck by block on lower left leg and received 3 sutures.

IE was assisting other miners build a solid block stopping at XC 16 between entries 1and 2 in HG 7. The wall was about 10 feet high when it started to fall over. IE had back turned to the wall getting another block when the wall fell. IE was struck by block on lower right leg causing a contusion.

Injured Employee and IE's buddy were taking bolter outby to wash. IE was taking bolter slack off rib when bolter cable got tight it jerked IE's arm. Causing IE forearm to swell and burn.

IE was using a 4' wrench to put up a cable bolt when the wrench shifted and came out of the drill chuck and hit IE in the forehead causing a laceration which needed 5 sutures.

IE was walking to truck, stepped on a piece of coal, and twisted EE's ankle.

IE slipped on a relay bar on the long wall face and EE's knee popped.

Operator was hauling jacks and shields with semi and lowboy. Upon arrival to mine in preparation for unloading they unsecured the jacks and one came at them pinning their leg to the ground. Truck and trailer where on level ground, jack was placed on cribs when loaded.

Cutting a zip tie and the knife went thru the zip tie and cut IE's finger. IE received 3 sutures.

Removing belt structure when a belt rail dropped and hit their knee causing a bruise. Employee first day off work was 10/27/2023.

Cutting belt with a knife. When knife came free, Injured Employee struck their other hand. Resulting in 6 stitches.

moving feeder when a roof jack extension fell off of the feeder and hit IE on the shoulder. Employee never missed a day of work until 9/26/2023.

IE was cutting banding when IE's hand came down on another piece of banding causing a laceration. IE's first day missed was on 09/15/2023.

Tightening bolts on a gear case and hurt upper back. Employee's first day off of work was on 09/11/2023.

A piece of rock fell from the top, hit the drum of the shear, and a piece of rock hit the IE in the mouth causing a laceration, IE received 7 sutures.

Pulling water hose up the slope and felt a pop in EE's shoulder

IE had hand on the back of the crib tie when another employee threw a crib tie smashing it between the crib IE was holding. IE received sutures.

Ie was lifting water line and felt a pop in EE's right elbow.

Picked up an oxygen bottle and sprained EE's shoulder.

Moving crib ties and IE felt EE's back popped. Accident was reported on O8/17/2023.

Banding belt structure when a piece of banding cut their arm. IE received 5 sutures.

Trying to clear structure from under 25X. IE was getting in 25X and a rail Struck their hand while closing the door.

Unrolling 3 inch poly pipe using the IT62. Was hooking the chains up to the rolls, there was a burr on the forks that cut left forearm. Received 5 sutures

IE was bending mesh with a partner when their shoulder popped. IE worked through 06/26/2023. IE first day off was on 06/27/2023.

Truck driver stepped down off the low boy trailer and stepped on to some chains that they had thrown off the trailer and twisted an ankle.

IE was taking mono rail down when it spun on EE hitting EE's ankle causing a contusion.

IE was building a kennedy wall when back started hurting. Dr. told IE to take some days off.

IE hit EE's finger with a hammer when capping in a crib tie causing a fracture.

Using a hammer to put a miner chain back together when a piece of metal came off the chain and hit IE in the forehead. IE received 1 suture.

Employee was vulcanizing on the dry refuse belt and when cutting the belt the knife slipped and contacted their right calf resulting in a laceration requiring 5 sutures.

IE was putting a 6 foot roof bolt up when EE's shoulder popped.

IE was working on the surface belt and stepped wrong twisting an ankle causing a fracture. Employee finished the shift and worked the next day. IE was scheduled the next 4 days off and went to Dr. on 04/17/2023 and found out they had a fracture.

IE was moving shear cable across the long wall face when IE slipped on the shield toes twisting their ankle.

IE was getting a 4 foot extension to add to the starter steel when the starter steel came out of the hole and hit IE left hand causing a fracture.

IE slipped in the mud at the longwall face and fell on a relay bar. IE was carrying a horizontal pin and EE fractured EE's finger when EE smashed EE's finger between the pin and the relay bar.

IE was hammering a knife valve closed when the hammer deflected off of the valve and struck IE finger causing a fracture. IE never missed a day of work and did not get x-rays until 1/23/2023.

IE was hanging line curtain when a piece of slate fell from the top and hit IE in the arm.

Mine manager found a roof fall in Headgate return in entry #2 at xc 53 which compromised a stopping between entry 1 and 2.

IE was holding a ball peen hammer trying to knock a pin on the ATRS when the coworker swung hammer hitting the ball been hammer a piece of metal came off the hammer and went into the IE arm.

IE was using the jaws of life and the cleat broke hitting IE in the nose fracturing it.

IE was using a chain binder and the binder slipped out of IE hand hitting the IE. IE strained back and neck when IE moved to get out of the way.

IE was ratchet strapping the flinger duster to the scoop when the hook slipped off the duster and struck IE in the face. IE received sutures.

IE was dragging tubes and felt something pull in their right calf.

IE was trying to get the razor blade out of EE's knife. The blade was stuck and when it broke lose the razor cut EE's thigh. IE received 3 sutures.

Walking on shield toes and slipped hurting their back.

Employee was performing belt work on the surface when EE began to have symptoms of heat exhaustion.

IE was stacking monorails on the trailer when it slid out of IE's hand smashing ring finger

IE was tossing belt rails when IE's hand hit the belt and the belt rail hit IE's hand against the belt causing a laceration which needed 5 sutures.

IE put hand on the pan line to brace themselves and a piece of coal came off the shear head hitting finger on the pan line causing a fracture.

IE slipped in the mud and hurt their back. *First day missed was 6/14/2022.

Employee was walking at the #5 Head Gate belt conveyor head roller and stepped onto a rock and experienced pain in the right foot.

IE put a bundle of plates on IE's partners tray, and put IE's hand on top of the fan to brace self. The EE rib pinning just finished putting in the rib bolt and when EE tilted the arm of the bolter back to the normal position the drill steel tray on the arm of the bolter hit IE's hand while it was on the fan causing a fracture on IE ring finger.

IE put a roof bolt in the top and when IE went to put the bolt in the chuck of the bolter the plate on the bolt slid down the roof bolt and cut EE's forearm. IE received 4 sutures.

IE was raking at belt head, turned sideways, and felt EE's knee pop.

IE was spreading the forks on the Mega-Trac and felt a pop in EE's stomach. The IE never missed a day of work until 05/11/2022. IE went to go have hernia surgery.

IE was taking down belt chain that was on the belt hangers when the chain swung around and hit IE in the mouth chipping IE front tooth.

IE was building the take up for HG3 drive and when IE went to set the rails for the take up, the rails slid on the bobcat smashing IE's thumb. IE received sutures.

IE stepped wrong off of a concrete pad spraining ankle.

IE was carrying belt structure and got foot caught between rollers and fell spraining ankle.

IE was pulling on a winch rope when it broke loose and IE fell back into the ramp pan. IE received 8 sutures.

Employee hit head in the cab of the dozer when the coal gave way by the feeder causing the dozer to go into the feeder. Employee received sutures.

IE tried lowering a shield that was metal bound with another shield. IE was standing on the feet of the shield and when the shield broke free it bounced and IE hit IE's head off the top of the shield hurting back. Accident Report was not filled out until 3-14-2022. IE worked from the date of the accident until 3-14-22.

Climbing down from a D10 dozer and EE's foot slipped on the ice covered step. Both of EE's hands were on the hand rails and EE's shoulder popped from hanging on the hand rails when their foot slipped.

IE was using channel locks to hold a piece of channel for another miner who was using a welder. The channel fell and hit the IE right thumb, causing a fracture.

EE was assisting with the installation of a pulley on the surface refuse belt. The pulley became bound up and a telehandler was being used to maneuver the pulley as to align the bolt holes. The pulley broke loose from the bolt that had been installed and shifted, striking the EE in the face.

Employee jumped off the pod duster and felt a pain in right hip/leg.

IE was putting miner parts on a pallet when IE felt something pop in lower abdomen. The IE had hernia surgery on 03/09/2022. The employee never missed a day of work from the accident to the surgery.

IE was drilling a hole, the starter steel got stuck in the top, and when they went to get another steel the starter steel fell from the top. The steel bounced off the drill pod and hit the IE on the forearm/wrist causing a hairline fracture.

Carrying a 72 inch top roller, tripped on a concrete block, and the roller smashed IE's thumb against the belt structure trailer.

IE was restacking belt rails when one belt rail slid off the pile and cut the end of a finger off at the first joint. The hand was on another belt rail when it got hit by the other belt rail.

IE was running the shear when the drum of the shear threw a rock and hit employee on the chin.

Operating a scoop heading inby battery first hauling cribbing. Scraped rib hit a rib strap that came into operators cab. Hitting I.E. in the right foot.

IE was crawling into the shield to set it when EE accidentally hit the pressure bleeding valve on the pressure whip line. The line shot back striking IE in the nose causing a fracture.

Employee was walking backwards from the forklift after putting a chain on the tugger and tripped over a crib tie. EE landed on the right hand/wrist and ended up with a fractured wrist.

Getting down from the back of the truck and EE's foot slipped off the back of the bumper causing EE to fall on their left shoulder.

Employee got crib mix in boot and received burns on top of EE's foot.

Employee went for carpal tunnel surgery on 04/06/2022

Employee was walking back to the truck and tripped over EE's own feet. When EE was falling EE put out EE's hand to catch self and ended up breaking a bone in EE's hand.

Employee was putting up bacon straps when EE's steel plugged and when EE pulled the steel out the bacon strap feel and hit EE on the head.

Employee was standing on the DMU jack plate and the FCT was moving when the guarding on the sprocket car went over employee's foot.

Employee was unhooking dust hose from the pod duster, EE did not realize that there was pressure still on the hose, and the end of the hose hit EE in the face.

Employee was pulling a hose on the pod duster and heard a pop in EE's shoulder.

Employee was drilling a hole and the steel was stuck in the top when EE realized had lost suction. EE put hand over the chuck to feel if had suction when the steel fell out of the hole had hit EE's hand causing a fracture.

a repairman was greasing the hopper on the FCT next to the landing on the DMU. The miner was backing up and the tail of the miner hit the hopper causing the hopper to move and pinched the repairman foot between the tire of the FCT and the railing of the DMU.

Employee was pulling belt structure when the belt rail fell and landed on EE's left foot right above the metatarsal causing a fracture.

Pulling shields up when some dust fell from the top and got in EE's eye. EE was prescribed with eye drops.

Was working on the belt line, a 2"" deluge line hit employee on the head on 2/10/2021. Was seen and released by a doctor on the same day. Had a follow up with family doctor on 2/15/2021 and was removed from work.

On 11/13/20 employee slipped on the stairs in the prep plant and went to catch self hurting shoulder. Employee never a missed a day of work until 7/8/2021 when employee had shoulder surgery.

On September 23 2020 employee hurt shoulder when taking out a water line from a tank on the water car and continued to work to October 28 2020. On October 29 2020 employee is having surgery done on the shoulder.

Employee was changing bits when a piece of rock from the corner of the face hit employee's hand that was on the head of the shear.

A piece of shale and coal fell from the longwall face bounced off of the chain and hit employee on the left foot.

Employee was putting a pin on the anti topple jack and employee pushed the jack out to line up with the hole when it smashed employee's finger.

Hose hit employee in the eye.

Stacking crib butts above their head and some wood chips fell down behind glasses and got in left eye.

Using a utility knife to cut a plastic tie and the knife went through the tie and cut the employee on the left lower leg. EE received 5 stitches.

EE was holding up connecting links so they didn't get fouled up and got the tip of EE's finger pinched.

During our weekly exam, the fire boss discovered a roof fall in the #1 Entry (Return), 4 Head gate, at XC 49.

Employee, was splicing car cable, using employee's utility knife pulling it towards self instead away from self. Employee received three stitches as a result of this accident to index finger on left hand.

EE was getting out of the Security vehicle and felt something give in EE's right hip area.

Trying to pull a thread out of a nut and cut finger and received 10 sutures.

The miner was lowering and moving a vertical Kennedy Stopping Panel when a small piece of top fell striking employee on the right side of employee's face (employee's cheek) beside of employee's nose.

AMS operator saw on the monitor that CO levels were over the spontaneous combustion plan at the mine.

A roof fall was discovered in the tailgate panel #4 travelway at 9:27pm. The fall was located at 61 crosscut intersection. The fall measured approx. 20 feet long and 10 feet in height. The fall had initially been bolted with 8' torque tension bolts. The limestone typically present in the anchorage zone of 8' bolt had thinned out in this area.

Small piece of slate fell and cut employees' lip.

While troubleshooting an oil leak on a can setter, employee moved a line and line exploded injecting oil into his index finer on right hand.

While cleaning around a broken shear pin, a rock fell smashing employee's little finger on left hand.

While cleaning off the top of the miner, a rock hit employee on his right foot.

Bent down to remove rock and felt pain in back. First day missed was 11/17/14.

While loading trash onto a trailer, a 2x2 board slipped and pinned employees leg inside cab of bobcat.

Rock hit employees hand while he was walking the pan line.

While tightening the tension spring on an airlock door, bar slipped causing tension dowel to spring back, smashing employees left index finger.

Found CO in the bleeders while doing weekly exams.

While employee was resting his arm on the miner, piece of rock fell hitting his right arm and thumb.

While changing oil, employee stepped in hole twisting left knee. First day missed was 11/4/14.

While employee was spreading loop on choker he popped his right shoulder.

Employee was freeing up mesh on the bolter and slipped, falling on the bolter. Employee returned to work with no restriction after seeing regular physician.

While walking to the face of Longwall, employee fell hitting right knee causing laceration that required stitches.

While on roof bolter, employee stepped out, lost balance and fell from bolter twisting knee and hitting back of foot.

While working on the roof bolter, employee got fingers caught in boom cylinder and drill body chain mount. Boom came down and severed 3 fingers on right hand.

A piece of rock hit employee on the thumb.

While clearing himself from pathway of buggy cable, operator turned on buggy and the cable caught employees' leg and flipped him.

A roof fall was discovered on the longwall Panel 2 belt line in 2 Headgate at 7:50 AM. The fall was located between 89 and 90 crosscuts. The fall measured approximately 50 feet long and 7 feet in height. The fall area had initially been bolted with 6' torque tension bolts. The limestone typically present in the anchorage zone of a 6' bolt had thinned out in this area.

Crew was advancing shield. Top rolled off shield onto mesh and the boat winch spun free. The handle hit employee in the face.

While placing a restraining clamp on shelf in warehouse, employee pulled lower back. First day employee missed work was 9/21/13.

While pulling an angle bracket out of mesh, bracket twisted and cut the employee on the left arm.

Employee was struck on left hand by hammer while hammering on winch rope hook.

Employee was filling duster with hose when it got plugged. Once the hose was cleared, employee was hit with hose.

Operator was changing bits when a piece of rock fell striking operator on top of the head.

Employee was standing under a longwall shield cleaning rocks from the walkway when an emulsion hose ruptured striking him in the right leg, right arm and right side of face (including eye).

Employee cut weld to release discharge. As discharge released, employees feet/legs were pinned between I-beam and discharge.

Loading supplies onto a bolter pinching hand between the thumb and finger.

Employee was using a roof bolt for a shaft in a coil of plastic pipe 1 1/2"" X 500' as pipe was being uncoiled the roof bolt came out and struck employee in the nose.

Using a utility knife, the employee was cutting a zip tie that was attached to a light cable.

Employee was cleaning trash off beltline and while loading trash into scoop, a metal band from a wooden pallet cut his forearm.

While bolting, steel drill hit left shoulder.
