IE was dropping tubes when the tube bounced off of the ground jamming EE's finger causing a fracture.

Torching out bolt off of a screen. The melted plastic popped. Sending a piece behind EE's glasses and striking EE in the eye. Had to get eye drops in eye.

Truck driver stepped down off the low boy trailer and stepped on to some chains that they had thrown off the trailer and twisted an ankle.

The EE rib pinning just finished putting in the rib bolt and when EE tilted the arm of the bolter back to the normal position the drill steel tray on the arm of the bolter hit IE's hand while it was on the fan causing a fracture on IE ring finger.
IE put a roof bolt in the top and when IE went to put the bolt in the chuck of the bolter the plate on the bolt slid down the roof bolt and cut EE's forearm. IE received 4 sutures.

IE was standing on the feet of the shield and when the shield broke free it bounced and IE hit IE's head off the top of the shield hurting back. Accident Report was not filled out until 3-14-2022. IE worked from the date of the accident until 3-14-22.
Climbing down from a D10 dozer and EE's foot slipped on the ice covered step. Both of EE's hands were on the hand rails and EE's shoulder popped from hanging on the hand rails when their foot slipped.
IE was using channel locks to hold a piece of channel for another miner who was using a welder. The channel fell and hit the IE right thumb, causing a fracture.
EE was assisting with the installation of a pulley on the surface refuse belt. The pulley became bound up and a telehandler was being used to maneuver the pulley as to align the bolt holes. The pulley broke loose from the bolt that had been installed and shifted, striking the EE in the face.
Employee jumped off the pod duster and felt a pain in right hip/leg.

a repairman was greasing the hopper on the FCT next to the landing on the DMU. The miner was backing up and the tail of the miner hit the hopper causing the hopper to move and pinched the repairman foot between the tire of the FCT and the railing of the DMU.
Employee was pulling belt structure when the belt rail fell and landed on EE's left foot right above the metatarsal causing a fracture.

On 11/13/20 employee slipped on the stairs in the prep plant and went to catch self hurting shoulder. Employee never a missed a day of work until 7/8/2021 when employee had shoulder surgery.
On September 23 2020 employee hurt shoulder when taking out a water line from a tank on the water car and continued to work to October 28 2020. On October 29 2020 employee is having surgery done on the shoulder.

A roof fall was discovered in the tailgate panel #4 travelway at 9:27pm. The fall was located at 61 crosscut intersection. The fall measured approx. 20 feet long and 10 feet in height. The fall had initially been bolted with 8' torque tension bolts. The limestone typically present in the anchorage zone of 8' bolt had thinned out in this area.

The fall measured approximately 50 feet long and 7 feet in height. The fall area had initially been bolted with 6' torque tension bolts. The limestone typically present in the anchorage zone of a 6' bolt had thinned out in this area.
