Employee was walking down stairs. Felt a pop in their left knee.

Struck by falling object

IE stepped on a sheet of metal and it flipped off of the step and landed on their foot causing a fracture.

IE was carrying a top belt roller when EE's foot got stuck in the mud. IE then fell and landed on the roller hurting EE's back.

The IE was moving the miner to the face in entry #3 when we found the IE pinned between the miner tail and the solid rib.

IE was pulling line curtain and twisted pulling a muscle in EE's back.

IE was using a pry bar to unplug a chute when a piece of rock rolled down the pile and hit the pry bar causing the pry bar to come down and smash a finger against the chute. IE received 6 stitches on ring finger.

IE stepped from one side of the bolter to the other and the left knee popped while checking the torque on the roof bolts.

IE was strapping down materials on the tool sled when a piece of pipe rolled off the top of the sled and smashed the right thumb causing a fracture.

IE was assisting another miner with removing the drill rig operator's platform so they could change the drill bit. The IE got a finger pinched between two pieces of chain, resulting in a laceration that required 4 stiches to close up

An unplanned roof fall occurred in entry #3 in the intersection of 79 in HG 8.

IE was dropping tubes when the tube bounced off of the ground jamming EE's finger causing a fracture.

IE was trying to get the mesh off the tip of the shield when the shield tip came down and hit IE in the back causing a fracture.

Went to change battery on a scoop when the battery slid off of the cribs and hit IE in the knee

IE was looking in the dust box when a rib rolled out and hit them into the bolter.

IE was using a pry bar when it slipped and smashed THEIR left index finger between the prybar and the miner.

IE went to connect the bottom drill steel with the top steel when the top drill steel fell from the top and smashed their little finger between the steels.

On the pre-shift examination for the oncoming 3rd shift, the examiner discovered evidence of a heating event in the vicinity of xc 26. The operator was able to contain the condition by loading it on the belt and sending it out of the mine.

IE slipped on a shield toe carrying a PMCR and fell hurting their back.

Allegedly a rock fell out of the face in entry #4 and hit IE in the shoulder knocking IE to the ground. IE landed on a rock and injured their knee.

The Headgate drum on the shear threw a rock out that hit IE on the back of left ankle.

IE was standing next to a piece of plastic pipe so it would not hit a truck that was by the slope drive. The pipe got caught on something in the slope and when it broke free the pipe hit swept IE off of EE's feet. IE received 6 sutures on EE's bottom lip.

IE was connecting the 2 drill steels and the steels got in a bind. IE hit the rotation level to get the two steels together and the bottom steel slipped off the top steel and hit IE in the nose.

IE was lifting a 4"" slick line up to hand it when EE heard EE's shoulder pop. IE never missed a day of work till 10/28/2024

Torching out bolt off of a screen. The melted plastic popped. Sending a piece behind EE's glasses and striking EE in the eye. Had to get eye drops in eye.

IE was cutting down wire mesh. The mesh fell and cut IE on the forearm. IE received 5 sutures.

They were reeling up the cable slack on the bolter when the cable got tight and hit IE in the legs causing EE to fall to the ground hurting their back.

Truck driver stepped down off the low boy trailer and stepped on to some chains that they had thrown off the trailer and twisted an ankle.

Employee was walking at the #5 Head Gate belt conveyor head roller and stepped onto a rock and experienced pain in the right foot.

IE put a bundle of plates on IE's partners tray, and put IE's hand on top of the fan to brace self. The EE rib pinning just finished putting in the rib bolt and when EE tilted the arm of the bolter back to the normal position the drill steel tray on the arm of the bolter hit IE's hand while it was on the fan causing a fracture on IE ring finger.

IE put a roof bolt in the top and when IE went to put the bolt in the chuck of the bolter the plate on the bolt slid down the roof bolt and cut EE's forearm. IE received 4 sutures.

IE was raking at belt head, turned sideways, and felt EE's knee pop.

IE was spreading the forks on the Mega-Trac and felt a pop in EE's stomach. The IE never missed a day of work until 05/11/2022. IE went to go have hernia surgery.

IE was taking down belt chain that was on the belt hangers when the chain swung around and hit IE in the mouth chipping IE front tooth.

IE was building the take up for HG3 drive and when IE went to set the rails for the take up, the rails slid on the bobcat smashing IE's thumb. IE received sutures.

IE stepped wrong off of a concrete pad spraining ankle.

IE was pulling on a winch rope when it broke loose and IE fell back into the ramp pan. IE received 8 sutures.

Employee hit head in the cab of the dozer when the coal gave way by the feeder causing the dozer to go into the feeder. Employee received sutures.

IE tried lowering a shield that was metal bound with another shield. IE was standing on the feet of the shield and when the shield broke free it bounced and IE hit IE's head off the top of the shield hurting back. Accident Report was not filled out until 3-14-2022. IE worked from the date of the accident until 3-14-22.

Climbing down from a D10 dozer and EE's foot slipped on the ice covered step. Both of EE's hands were on the hand rails and EE's shoulder popped from hanging on the hand rails when their foot slipped.

IE was using channel locks to hold a piece of channel for another miner who was using a welder. The channel fell and hit the IE right thumb, causing a fracture.

EE was assisting with the installation of a pulley on the surface refuse belt. The pulley became bound up and a telehandler was being used to maneuver the pulley as to align the bolt holes. The pulley broke loose from the bolt that had been installed and shifted, striking the EE in the face.

Employee jumped off the pod duster and felt a pain in right hip/leg.

IE was putting miner parts on a pallet when IE felt something pop in lower abdomen. The IE had hernia surgery on 03/09/2022. The employee never missed a day of work from the accident to the surgery.

IE was drilling a hole, the starter steel got stuck in the top, and when they went to get another steel the starter steel fell from the top. The steel bounced off the drill pod and hit the IE on the forearm/wrist causing a hairline fracture.

Carrying a 72 inch top roller, tripped on a concrete block, and the roller smashed IE's thumb against the belt structure trailer.

IE was restacking belt rails when one belt rail slid off the pile and cut the end of a finger off at the first joint. The hand was on another belt rail when it got hit by the other belt rail.

Employee was walking backwards from the forklift after putting a chain on the tugger and tripped over a crib tie. EE landed on the right hand/wrist and ended up with a fractured wrist.

Employee was walking back to the truck and tripped over EE's own feet. When EE was falling EE put out EE's hand to catch self and ended up breaking a bone in EE's hand.

Employee was putting up bacon straps when EE's steel plugged and when EE pulled the steel out the bacon strap feel and hit EE on the head.

Employee was standing on the DMU jack plate and the FCT was moving when the guarding on the sprocket car went over employee's foot.

Employee was unhooking dust hose from the pod duster, EE did not realize that there was pressure still on the hose, and the end of the hose hit EE in the face.

Employee was drilling a hole and the steel was stuck in the top when EE realized had lost suction. EE put hand over the chuck to feel if had suction when the steel fell out of the hole had hit EE's hand causing a fracture.

a repairman was greasing the hopper on the FCT next to the landing on the DMU. The miner was backing up and the tail of the miner hit the hopper causing the hopper to move and pinched the repairman foot between the tire of the FCT and the railing of the DMU.

Employee was pulling belt structure when the belt rail fell and landed on EE's left foot right above the metatarsal causing a fracture.

Pulling shields up when some dust fell from the top and got in EE's eye. EE was prescribed with eye drops.

Was working on the belt line, a 2"" deluge line hit employee on the head on 2/10/2021. Was seen and released by a doctor on the same day. Had a follow up with family doctor on 2/15/2021 and was removed from work.

On 11/13/20 employee slipped on the stairs in the prep plant and went to catch self hurting shoulder. Employee never a missed a day of work until 7/8/2021 when employee had shoulder surgery.

On September 23 2020 employee hurt shoulder when taking out a water line from a tank on the water car and continued to work to October 28 2020. On October 29 2020 employee is having surgery done on the shoulder.

Employee was changing bits when a piece of rock from the corner of the face hit employee's hand that was on the head of the shear.

A roof fall was discovered in the tailgate panel #4 travelway at 9:27pm. The fall was located at 61 crosscut intersection. The fall measured approx. 20 feet long and 10 feet in height. The fall had initially been bolted with 8' torque tension bolts. The limestone typically present in the anchorage zone of 8' bolt had thinned out in this area.

A roof fall was discovered on the longwall Panel 2 belt line in 2 Headgate at 7:50 AM. The fall was located between 89 and 90 crosscuts. The fall measured approximately 50 feet long and 7 feet in height. The fall area had initially been bolted with 6' torque tension bolts. The limestone typically present in the anchorage zone of a 6' bolt had thinned out in this area.
